Dear Mr. Slawinski:
i In response to your letter dated on March 1,
1993 1 am not contesting any of the violations you have listed.
I' hope this response clears up any questions you might of had on the last 1
response.
(1) Failure to properly check al ar m ratemetere pr ior to use for proper function.
(a) Reason:
The employee James Locklin told me he knew how to use and l
operate this device.
I assumed he knew how as he worked for American Testing in Indiana and had to use them before te came'to work for me.
(b) Corrective action taken:
j This emp1oyee and al1 others were instructed on the proper use and testing of these ratemeters.
(c ) Corrected measures to make sure this does not happen again.
Once overy month everyone is asked to show me how to test and use the ratemeter.
They are al1 shown how e f ter they show me.
(3) Information on shipping papers-and proper 1y marking transport packages.
l (a) Reason:
The container in the truck was marked and I was on the i
understanding,that was the transport centainer.
The information on the chipping papers was thought to contain all necessary information.
(b) Corrected action taken:
Al1 transport containers and packages have "RG" stamped on them.
All shipping papers have the same and are all visible.
(c) Cor r ec t ed steps to make sure this does not happen again When a new source comes in the shipping papers are properly taken care of.
As for the transport packages we don not at this time see buying any more of them.
But if we do they will be properly labeled at the time they arrive.

(a) Reason:
Every exposure device that I have has Amersham's name, address, and phone number on them.
I was on the understanding that that was all that was req 1 red.
(b) Corrected action taken:
I have tags with my company name, address, and phone number on them attached to each exposure device.
(c) Corrected st eps taken to make sure this does not happen again:
I have extra tags made up in case one gets torn off or new equ i pnien t Is purchased.
