50-458 River Bend Station License: NPF-47 1
During an NRC inspection conducted on March'8-12, 1993, two violations of NRC requirements were identified. The violations involved a failure to adequately account for keycards and a failure to maintain an uninterruptible power supply to the security systems.
In accordance with the " General Statement of Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violations are listed below:
Condition 2.D of the River Bend Station Operating License NPF-47 requires that the licensee maintain in effect and fully implement all provisions of the Commission approved Physical Security Plan. This includes amendments and changes made pursuant to the authority of 10 CFR 50.54(p).

Failure to Maintain Keycard Accountability The Introduction Section of the Physical Security Plan states that security procedures provide the detailed instructions necessary to implement the plan.
Security Position Instruction, Procedure SPI-12, paragraph 3.2.4(1) states, in part, " Supervise the inventory of all KCs (Keycards) and VKCs (Visitor Keycards) as follows:" Paragraph 3.2.4(1)(i) states, "Using the SUP1 console, log out KCs displayed as [in] the PA but in the rack.
Prepare a SECFM 115, Security Incident Report."
Contrary to the above, the inspectors identified that on August 24, 1992, and March 1,1993, keycards were displayed as being in the protected area when, in fact, the badges were in the security badge racks. The keycard inventory did not identify the discrepancy in the keycard accountability program.
This is a Severity Level IV violation (Supplement III) (458/9308-01).

Failure To Maintain Uninterruptible Power to the Security-Systems-i l
Paragraph 7.5 of the Physical Security Plan requires that-security equipment be furnished uninterruptible power from Suses that are also backed by standby diesel generators.

Contrary to the above, the inspectors identified that from July 22. until; i
August 4, 1992, the uninterruptible power-system was in manual bypass, j
thereby, eliminating uninterruptible power for the system.
i This is a Severity Level IV violation (Supplement III)'(458/9308-02).
Pursuant to the provisions of 10 CFR-2.201, Gulf States Utilities. is hereby; j
required to submit a written _ statement or explanation to the Regional j
Administrator, Region IV, with a copy to the U.S. Nuclear' Regulatory 1
Commission, ATTN: Document Control Desk, Washington, D.C. 20555, within 1
30 days of-the date of the letter transr,itting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of-Violation" and should include for each violation:
(1) the reason for the violation, or, if contested,~ the basis for_ disputing the: violation, (2) the
.j corrective steps that have been _taken and the results achieved, (3) the 1'
corrective steps that will be taken to avoid further violations,'and (4) the date when full. compliance will be achieved.
If an adequate reply-is not received within the time specified in this Notice, the Commission may. issue an Order or Demand for Information as to why such other action as 'may be proper 1
should not be taken. Where good cause is shown, consideration will_be given-
]
to extending the' response time.
