VIRGINIA ELECTRIC AND POWER COMPANY NORTH ANNA POWER STATION UNIT 1 VOLUNTARY SPECIAL REPORT FOR INOPFJABLE FIRE STOPS IN CABLE TRAYS FOR GREATER THAN 14 DAYS i
inspections were completed during the current No.-th Anna Unit 1 outage for 10 CFR 50 Appendix R fire stops in the Unit 1 containment. The inspections identified seven cable tray fire stops which were not properly installed. This voluntary report is being submitted because these cable tray fire stop conditions appear to have been in existence for at least one refueling cycle, and some may have existed since initial instaliation due to inadequacies in the surveillance procedure.
An initial inspection of Appendix R cable tray fire stops in the Unit 1 containment was conducted on February 16,1993. The inspection used revised acceptance criteria in the fire barrier inspection procedure. The acceptance criteria had been revised to address NRC concerns with fire barrier inspection procedures.
The application of the revised acceptance criteria during this inspection revealed two cable tray fire stops which had not been properly installed and which required rework.
The inspectors, in conjunction with engineering, also identified several Appendix R fire stops which were not uniquely identified by the inspection program and are not listed in the North Anna Power Station 10 CFR 50 Appendix R Report (e.g., several trays have two firestops, but only one was identified in the procedure). Further inspections were conducted on February 18,1993, to inspect the cable tray fire stops in Unit 1 containment that were not included in the surveillance program and identified five additional cable tray fire stops which were not properly installed.
1 Corrective maintenance work orders were submitted to repair all improperly installed cable tray fire stops in the Unit 1 containment. These repairs will be completed prior to returning Unit 1 to above mode 5.. The North Anna Power Station 10 CFR 50 Appendix R Report will be revised to address the fire stops issues that were identified during this inspection process. Inspection procedures will be updated to include all required fire stops and to reflect the labeling of Appendix R cable trays in Unit 1 containment.

l These conditions did not result in any significant safety consequences, because the affected fire stops were partially in place and would have impeded flame spread along -
the cable trays. Also, fire detection equipment was operational during this surveillance period.
The Unit 2 containment Appendix R cable tray fire stops will be inspected for similar concerns during the Unit 2 refueling outage scheduled for September,1993.
This Special Report has been reviewed by the Station Nuclear Safety and Operating Committee and will be provided to the Management Safety Review Committee.
