Dear Mr. Orser:

This refers to the routine safety inspection conducted by Messrs. W. J. Kropp,

K. Riemer, and R. Twigg of this office from January 19 through March 9, 1993.

The inspection included a review of activities at your Fermi 2 facility

At

the conclusion of the inspection, the findings were discussed with those

members of your staff identified in the enclosed report.

Areas examined during the inspection are identified in the report.

Within

these areas, the inspection consisted of a selective examination of procedures

and representative records, observations, and interviews with personnel.

During this inspection, certain of your activities appeared to be in violation

of NRC reportability requirements of 10 CFR 50.72, as specified in the

enclosed Notice of Violation. A written response is required. The violation

is of concern because your staff used draft reportability guidance not yet

approved by the NRC staff in concluding an ESF actuation was not reportable.

In addition, another violation was identified that met the requirements of 10 CFR Part 2, Appendix C, Section Vll.B; therefore, a Notice of Violation will

not be issued.

Also, an Unresolved Item documented in paragraph 6.b of this report was is. sued

that pertained to recent failures of the High Pressure Coolant Injection

(HPCI) system during surveillances. These failures are of concern to the NRC

staff since the Fermi 2 Individual Plant Examination identified the HPCI

system as important in contributing to a low core damage frequency.

Your

staff is requested to identify in writing those actions that have been or will

be taken to improve the reliabi'ity of HPCI.

As agreed upon with your staff,

a written response is due May ?

1993.

In accordance with 10 CFR 2.790 of the Commission's regulations, a copy of

this letter, the enclosed inspection report, and your response will be placed

in the NRC Public Document Room.

The responses directed by this letter and the accompanying Notice are not

subject to the clearance procedures of the Office of Management and Budget as

required by the Paperwork Reduction Act of 1980, PL 96-511.
