i As a result of the inspection conducted from January 26 to January 29,1993, and in accordance with the " General Statement of Policy and Procedure for NRC Enforcement

Actions," 10 CFR Pan 2, Appendix C (Enforcement Policy) (1993), the following violations were identified i

10 CFR 150.20 provides that persons who hold a specific license from an Agreement State are granted a NRC general license to conduct the same activity in a non-Agreement State provided the general licensee complies with the provisions of a specific license issued by the Agreement State.

Condition 17 of Maryland License No. MD-31-025-03 requires, in part, that no teletherapy unit source transfer shall be initiated unless the licensee has available a

manufacturer's written instructions or equivalent as outlined in Neutron Products' l

Specification P-9 and the tools for the opemtion, such instructions and tools being-l specific for each make and model of teletherapy unit and for the type of operation l

involved.

f Neutron Products Specification P-9 requires, in part, that wrist dosimeters be worn by individuals transferring a teletherapy source from a teletherapy treatment unit to a shipping container.

Contrary to the above, on January 26,1993, wrist dosimeters were not worn during the transfer of a teletherapy source. Specifically, an individual transferred approximately 2900 curies of cobalt-60 from an AECL Theratrun 80 teletherapy unit at the Lankenau Hospital into a shipping container without wearing wrist dosimetry.

Condition 18 of Maryland License No. MD-31-025-03 requires that licensed materials be possessed and used in accordance with statements, representations and

procedures contained in an application dated April 29,1981.

l Procedures in this application require, in pan, that pocket dosimeters be calibrated on a semi-annual basis.

Contrary to the above, as of January 26,1993, pocket dosimeters used by individuals performing licensed activities were not properly calibrated. Specifically, pocket dosimeters with Serial Nos. 1014098 and 8100015 were last calibrated in 1990 and 1991, respectively, a period greater than semi-annually.

This is a Severity Level IV violation. (Supplement VI)

I Pursuant to the provisions of 10 CFR 2.201, Neutron Products, Inc. is hereby required to submit to this office within thirty days of the date of the letter which transmitted this Notice, 7

a written statement or explanation in reply, including: (1) the cormctive steps which have i

been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Where good cause is shown, consideration will be given to extending this response time.
