Shembarger and
circumstances that led to the Unit I and Unit 2 HPCI system check valve -
leakage that was identified on February 5,1993, at the Quad Cities Nuclear
'
Plant and the impact the leakage had on HPCI system operability. At the
conclusion of the inspection the findings were discussed with those members of
c'
your staff identified in the enclosed report.
The enclosed copy of our inspection report identifies areas examined during
the inspection. The inspectors selectively examined procedures and
representative records, made observations, and conducted interviews with your
.'
personnel.
The inspectors determined that the HPCI systems were in a degraded condition
for several years as a result of the check valve leakage, which could have
affected the ability cf these systems to perform their function during a small
break LOCA event. The inspector identified several weaknesses that led to the
misaligned hinge and disk assemblies, including improper check valve
classifications in the IST program, inadequate check valve testing, and an
'
inadequate maintenance procedure.
Based on the results of this inspection, certain of your activities appeared
to be in violation of NRC requirements as specified in the enclosed Notice of
i
Violation (Notice).
You are required to respond to this letter and should follow the instructions

specified in the enclosed Notice when preparing your response.
In your
l
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your resonse to this
Notice, including your proposed corrective actions and the results of' future
inspections, the NRC will determine whether further NRC enforcement action is
necessary to ensure compliance with NRC regulatory requirements.
