NOTICE OF VIOLATION South Carolina Electric & Gas Docket No. 50-395 V. C. Summer License'No. NPF-12 During an NRC inspection conducted on January 25-29, 1993, violations'of_ NRC requirements were identified.

In accordance with the " General Statement of-Policy and Procedure for NRC Enforcement Actions," 10 CFR Part 2, Appendix C, the violations are listed below:

10 CFR, Part 50, Appendix B, Criterion V states in part,... Instructions, procedures, or drawings shall include appropriate quantitative'or qualitative acceptance criteria for determining that important activities-have been satisfactorily accomplished.

The licensee's quality assurance program chapter 13.5.1.12, states that the test procedures used to demonstrate that an item will perform satisfactorily in service are accomplished properly. Test procedures may incorporate or reference the requirements and acceptance limits contained in applicable design documents.

Contrary to the above, on January 27,1993, two Electrical Maintenance Procedures and one Preventive Test Procedure were-found to contain inadequate acceptance criteria as described below.

Electrical Maintenance Procedure EMP-445.0ll, BASELINE TESTING 0F LIMITORQUE VALVES WITH THE M0 VATS 3000 SYSTEM.

The purpose was to analyze the general mechanical condition and electrical set-up_ of Limitorque motor operated valves. The acceptance criteria required the results to be reviewed by the MOV coordinator but listed no data for the reviewer to base acceptance.

Electrical Maintenance Procedure EMP-445.012, DIFFERENTIAL PRESSURE TESTING OF LIMITORQUE VALVES. The purpose was to provide a guide for the differential pressure testing of Limitorque motor operated valves.

The acceptance criteria required that testing and analysis has verified that the valve will open and close fully against differential pressure. There is no value given for what differen-tial pressure must be met or where the required pressure is to be obtained.

Preventive Test Procedure PTP-270.001 through 270.015, DP TESTING OF (Valve numbers for valves in the MOV program). The purpose of_this procedure was to establish system conditions for flow and differ-ential pressure. testing of Generic Letter 89-10 M0V's. The accep--

tance criteria requires that the valve indicate closed by position indication lights, or diagnostic DP test signatures'and' meet the.

acceptance criteria of EMP-445.012 which is discussed above. There is no requirement that flow shut off be verified.

Summer License No. NPF-12 Pursuant to the provisions of 10 CFR 2.201, South Carolina Electric & Gas Company is hereby required to~ submit a written statement or explanation to the U.S. Nuclear Regulatory Commission, ATTN: Document Control Desk, Washington, D.C. ~ 20555, with a copy to the' Regional Administrator, Region II, and a copy to the NRC Resident Inspector at the facility that is the subject of this Notice, within 30 days of the date of the letter' transmitting this Notice of Violation (Notice). This reply should be clearly marked as a " Reply to a Notice of Violation" and should include for each violation:

(1) the reason for the violation, or, if contested, the basis for disputing.the violation, (2) the corrective steps that have been taken and the results achieved, (3) the corrective steps that will be taken to avoid further violations, and (4) the date when full ' compliance will be achieved.

If an adequate reply is not received within the time specified in this Notice, an order or Demand for Information may be issued as to why the license should not be modified, suspended, or revoked, or why such other action as may be proper should not be taken. Where good cause is shown, consideration will be given to extending the response time.
