NRC INSPECTION REPORT 50-445/93-08; 50-446/93-08 (NOTICE OF

VIOLATION)

This refers to the inspection conducted by Mr. P. M. Madden of the Office of

Nuclear Reactor Regulation and the inspectors identified in the enclosed

report on January 11-15, 1993. The inspection included a review of activities

authorized for the Comanche Peak Steam Electric Station, Units 1 and 2,

facility. At the conclusion of the inspection, the findings were discussed

with Messrs. Ji. D. Bruner and C. L. Terry and those members of your staff

identified in the enclosed report.

Areas examined during the inspection included followup on inspection findings

identified during the Fire Protection Team Inspection conducted

November 16-20, 1992, and an assessment of your procurement, training, quality

assurance, engineering, installation, and testing activities associated with

your use of Thermo-Lag as a fire barrier system.

Within these areas, the

inspection consisted of selective examinations of procedures and

representative records, interviews with personnel, and observation of

activities.in progress.

Based on th'e results of this inspection, certain of your activities appeared

to be in violation of NRC requirements, as specified in the enclosed Notice of

Violation (Notice). The violation is of concern because a failure in the work

control process resulted in a significant percentage of your safe shutdown

'

emergency lighting being inoperative, after the system had been turned over to

the operations organization as functional. The inoperative safe shutdown

lighting may not have been detected until the performance of the next

scheduled preventive maintenance, which occurs at 6-month intervals.

Three unresolved items and two inspection followup items, from the initial

inspection, pertaining to:

the susceptibility of certain safe shutdown motor-

operated valves to be rendered inoperable due to fire induced short

circuiting, as discussed in NRC Information Notice 92-18; testing and

maintenance of emergency lighting; positioning of smoke detectors; physical

location of sprinkler heads; and completion of the high-impedance fault study

for Unit 1, were closed based on your commitments to take appropriate actions

,

in a timely manner.

During this inspection, further commitments were made

,

pertaining to justifications for Thermo-Lag configurations which deviate from

tested configurations and completion of 100 percent of the preventive

maintenance procedures for emergency lighting. All of these commitments are

docketed in your letters dated December 23, 1992,- and January 15, 1993, which

are provided as Attachments 2 and 3 to the enclosed report.

You are required to respond to this letter and should follow the instructions

't

specified in the enclosed Notice when preparing your response.

In your

response, you should document the specific actions taken and any additional

actions you plan to prevent recurrence.

After reviewing your response to this

'

Notice, including your proposed corrective actions and the results of future

inspections, the NRC will determine whether further NRC enforcement action is

necessary to ensure compliance with NRC regulatory requirements.

In accordance with 10 CFR Part 2.790 of the NRC's " Rules of Practice," a copy

of this letter and its enclosures will be placed in the NRC's Public_ Document _

Room.

The responses directed by this letter and the enclosed Notice'are not subject

to the clearance procedures of the Office of Management and Budget, as

required by the Paperwork Reduction Act of 1980, Pub. L..No. 96.511.
