Dear Mr. Orlando:
i 1 am the Technical Director - Property Claims at American Nuclear insurers. I am currently i
looking into the merits of a claim from Cintichem for recovery of costs associated with l-131 contamination and subsequent decontamination which manifested itself in October,1989.
i The purpose of my letter is to request from you and the NRC any information that may be i
available that would shed light on the following questions:

What were the sampling points and the results of those samples taken before the 1989 j
contamination?

What were the sampling points and the results of those samples taken after the 1989 contamination?

What was the source of 1-131 discovered in S-4 in October,1989?

What corrective action did Cintichem take?

What other paths of contamination were identified, if any, and what was done about j
them?

Has any contamination been identified since Cintichem ceased operations? '
