SUBJECT: NRC INSPECTION OF THE TROJAN NUCLEAR PLANT AND MANAGEMENT

MEETING WITH LICENSEE ON MARCH-5, 1990

This report refers to the special unannounced inspection by Mr. D. B. Pereira-

of this office on February 26, 1990 through March 2, 1990, of activities

authorized by NRC License No. NPF-1.

Findings were-discussed with members of

your staff at the conclusion of the inspection on March 2, 1990.

In addition,

this report addresses the issues that were discussed in the management meeting

heldintheRegicaVofficesonMarch5,.1990withTrojanManagementand

Region V personnel.

Areas examined during this insaection are described in the enclosed inspection

report.

Within these areas, t1e inspection consisted of selective

examinations of )rocedures and representative records,-interviews with

t

personnel, and caservations by the inspector.

Based on the results of this insaection, no violations or deviations of NRC

P

requirements were identified wit 11n the scope of this inspection. ' However,

your implementation of the 1989 Requalification Program was deficient with

i

respect to grading the written examinations of licensed operators'.

Your

'

original regrading process, conducted by a single individual for both review

and approval, had limited management involvement and procedural controls.

It is an encouraging sign'that this weakness was identified by your Quality

.

o

Assurance Department, in an audit of the Requalification Program in January

i

'

1990. This audit determined that four licensed operators', originally passed

l

by PGE, should have failed their 1989 requalification examination. 'This

.

.

effective self-criticism by PGE significantly mitigates the program deficiency

l

l

identified.

Further, at the management meeting on March 5, 1990, PGE adequately addressed

l

the root cause of this incident, namely insufficient' procedural controls and

direct management involvement.

You confirmed that all four operators who were

incorrectly passed had been removed from further licensed duties until they

had passed a comprehensive annual examination as required.

The NRC will follow up on your corrective actions during a previously

scheduled NRC requalification examination to be conducted in April,1990.

NRC has requested 5-6 individuals out of the 11 original failures be examinea

-

!

in the April, 1990 Requalification Program and that the three individuals who

failed both (or failed one and nearly failed the other, in one case) the 1988

and 1989 requalification exams be examined again in the 1990 NRC examination.

In accordance with 10 CFR 2.790(a), a copy of this letter and the enclosure

will bt- placed in the NRC Public Document Room.

No response to this letter is required, but should you have any questions

concerning this inspection,-we will be pleased to discuss them with you.
