Subject: ' Inspection Report No. 50-289/89-82

This letter _. transmits'the report of a special maintenance team inspection led
by Mr. Walter Baunack of this office from December 4-15, 1989,. and January
22-25,'1990 at the:Three Mile Island Unit 1 Nuclear Station at Middletown,
Pennsylvania. Mr. Baunack discussed the findings.of the inspection with you
-
and members of your staff on December 15, 1989 and January 25, 1990;
This inspection was performed as part of-the NRC's industry wide effort to
evaluate the effectiveness of maintenance activities at licensed power reactors.
,
The team. concluded that Three Mile Island Unit 1 has implemented a generally
effective maintenance program. A number of strengths and weaknesses were
identified ~ by the-team as detailed in the enclosed report.
The team identified
a concern that in specific instances there was incomplete documentation of-work
activities such as tagouts, post maintenance testing, and completed' work: packages,
'
nor were certain activities such as control =of contractors documented in-
procedures. - In general, this lack of documentation was overcome by atcapable
'
staff, good management, and good implementationLof the'se activities.
Based on the results of-this inspection, it appears that one of-your activities
"
was not conducted in full compliance with-NRC requirements as s'et forth in the
Notice of Violation, enclosed herewith as Appendix A. 'The violation,has been
categorized by severity level in accordance with the General Statement of Policy-
a
and Procedure for NRC Enforcement Actions,10 CFP. Part 2, - Appendix: C.
-You are
required to respond to this Notice of Violation and in: preparing your reseonse,
you should follow the instructions in Appendix A.
We request that you notify-us in writing of actions taken-or' planned in order
to enhance maintenance activities relative to the weaknesses described in
'
Attachment 3.
Please provide this response within'60 days of receipt of this
letter.
