ATTACHMENT FOR ITEM 1:

The scope of the amendment is to change the locatisn where the licensed material is used. The location change is from:

18530 South Miles Parkway Cleveland, Ohio 44128 to:

Edison Park 8499 Darrow Road Twinsburg, Ohio 44087 The General Electric Company has purchased Reuter-Stokes, Inc.

and will move the operation to Edison Park in Twinsburg, Ohio.

The new location is about 15 miles southeast of the present location, Warrensville Heights, Ohio, a southeastern suburb of Cleveland.

The name of the corporation, Reuter-Stokes',

Inc., the place of incorporation,

Delaware, and the officers of the corporation, remain the same. The corporation is being run as a subsidiary of The General Electric Company through its Nuclear Energy Business Operation in San Jose, California.

ATTACHMENT FOR ITEM 13:

The laboratory facilities at the Edison Park location are shown in the attached Fig. I and 2.

Shielding for the C0-60 source is similar to that used at the present facility, and is shown in Fig. 3 & 4.

ATTACHMENT FOR ITEM 15:

Radiation Measurements Corporation of Chicago, Illinois, or another firm with acceptable qualifications, will be contracted to move the by-product material from the Warrensville Heights facility to the Twinsburg location. After the Warrensville Heights facility has been vacated, Reuter-Stokes will decontaminate any applicable areas and then contract Radiation Measurements Corporation to j

perform a decommissioning and decontamination survey. The Radiation Safety Program and the commitment by Reuter-Stokes to the program in order to achieve dose rates that are as low as reasonably achievable, remains the same.

All Radiation Safety procedures, directives, and controls apply to the new location.
