b; APPENDIX A NOTICE OF VIOLATION hhiokol Chemical Corporation License No. 01-00856-02
-As a result of the inspection conducted on September 23, 1981, and in accordance with the Interim Entorcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.
A.
10 CFR 34.28b requires in part that records of inaections and maintenance of radiographic exposure devices, storage container s and source changers be kept for two years.

Contrary to the above, the requirement to maintain records was not met in that daily inspections of equipment have been performed but no records have been kept.
This is a Severity Level VI Violatfor. (Supplement VII).
B.
License Condition 15 requires that licensed material be used in accordance with statements and procedures contained in your application dated June 6, 1978. Paragraph 3.9.4 requires that an acnual facility survey be conducted
' of Building 7345 with a maximum strength source unshielded in the center of each bay.
Contrary to the above, the requirement to. perform annual facility surveys 1
was not met in that no annual facility surveys are being perforned.
This is a Severity Level V.Violatt'on (Supplement VII).
C.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
The responses directed by this Notice are not subject to the clearance procedures of the Of fice of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
