ARIZONA PUBLIC SERVICE COMPANY (APS)

PVNGS UNIT 1 I.

Description of Deficiency During Unit 1 Steam Generator tube eddy current exsmination, the "A" Steam Generator had eighty-one (81) blocked tubes and the "B" Steam Generator had nine (9) blocked tubes preventing passage of the eddy current probe. The obstructions were from felt cleaning plugs inadvertently left inside the steam gener-ator tubes during cleaning operations in the CE shop. The subject felt plugs were subsequently removed by means of com-pressed air.

II.

Analysis of Safety Implications This condition is evaluated as being Not Reportable.

The following factors were considered in reaching this conclusion:

1.

One of the requirements in the selection of the material for the cleaning plugs was that the material dissolve when exposed to primary coolant chemistry and temperature. Tests have shown that the felt plugs will completely decompose at 5000F in the water c.hemistry of the pre-core test program.

2.

CE has determined t hat the decomposition products of the number of felt plugs reported will have no harm-ful effects, especially when the residue is diluted by the large inventory of primary wates.

3.

The effects of felt plug decomposition products on the reactor core need not be addressed, since the fuel will not be loaded until after hot-functional testing, by which time the decomposition products would have been removed from the system.

Based on the above information, it is concluded that the plugs left inside the steam generator tunes could not have adversely affected the safety of operation of the plant, therefore this situar. ion is Not Reportable in accordance with 10CFR50.55(e) which addresses safety implications of manufacturing deficiencies assuming they go uncorrected.

III.

Corrective getion The blocked tubes have been cleared by removing the felt plugs.
