SCOPE: This inspection was made as a result of the issuance of a 10 CFR Part 50.55(e) Construction Deficiency Report, pertaining to design and fabrication deficiencies in HS-142 beam attachments that have been furnished to the Washington Public Power Supply System Nuclear Project 2.

Areas selected for inspection included welding material control, joint fituo and welding, manufacturing process control and control of nonconformances and corrective action.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Quality Assurance Procedure H-QA-7, Revision 3, " Manufacturing Process," dated May-1,1979, Sho;. Fabrication Sheet Drawing No. 3.107.100.417 for a 6 inch schedule 40 pipe, 21 inches long, Mark No. 2CCP-PSSP047, was not signed or dated.

Contrary to Criterion V of Appendix B to 10 CFR Part 50 and the QC note on Shup Fabrication Sheet No. 317005, Revision 0, the heat number had not been stamped on 1/2 of item K which was a part of hanger Mark No. 1E61-H005 (Authorized No. 5280).

Contrary to Criterion V Appendix B to 10 CFR Part 50 and NA-4451 in Section III of the ASME Code,1t could not be ascertained from records that welding on shop fabrication sheet No. 3-082-071-027 had been accomplished by qualified personnel using qualified procedures, in that the welder or the welding procedure specification used cCJld not be identified.

4 Contrary to Criterion V of Appendix B to 10 CFR Part 50 and Stone and Webster Specification No. 2BVS-59, it could not be ascertained that the required visual examination of welds had been accomolished, in that the shop fabrication sheet No. 3-082-071-027 did not require or have pro-visions for visual examination of completed welds.

Contrary to Criterion V of Appendix B to 10 CFR Part 50. and shop fabrication sheet 3-082-071-027-8, fillet welds were found to be approximately 1/16 inch under the required 1/2 inch minimum.

OTHER FINDINGS OR COMMENTS:

Follow uo on 10 CFR 50.55(e) Reoort Power Piping Company has changed the design of the HS-142 beam attachment and it is now identified as the M-146 beam attachment.

The M-146 beam attachments sizes 15 through 40 are machined from SA-36 material and sizes 50 through 80 are fabricated with partial penetration welds which provides adequate strength, using ASME Code allowables, to sucport the design load rating at any angle off perpendicular.

PAGE 3 of 3 Power Piping Company, has supplied the HS-142 welded type beam attach-ments that may have a design deficiency to the following Nuclear Power Plants:

(a)

Crystal River, Unit 3 Florida Power Corporation (b)

Enrico Fermi, Unit 2 Detroit Edison (c)

Beaver Valley, Unit 2 Duquesne Light Company (d)

Perry Units 1 and 2 Cleveland Electric Illuminating The Power Piping Company has derated the load capacity of the HS-142 beam attachment for WPPSS No. 2 and completed a review of the loading of beam attachments supplied to Perry Nuclear Power Plant Units 1 and 2 and noti-fied Gilbert Associates, Inc. in a letter dated November 6, 1980, of those beam attachments that may be overicaded.

As of the date of this inspection Power Piping Company has not notified the three remaining nuclear power plants of the design deficiency of the HS-142 beam attachments.
