m Power Piping Company Docket No. 99900701/81-01 NOTICE OF NONCONFORMANCE Based on tha results of an NRC inspection conducted on June 29 - July 2,1981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawinos, of a type appropriate to the circumstances and shall be accomp-lished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished." Nonconformances to these require-ments are as follows:
A.
Paragraph 7.1.1.1 of Quality Assurance Procedure (Authorization No. 4200-N)
No. H-QA-7, Revision 3, " Manufacturing Process," dated May 1, 1979, states, "The QC Inspector shall sign-off and date the QC copy of the Shop Fehri-cation Sheet and fill in the Ledger Control Number in the appropriate blocks in the Bill of Material, when full traceability is required."
Contrary to the above, under Authorization No. 4200-N the Shop Fabrication Sheet Drawing No. 3.107.100.417 for a 6 inch schedule 40 pipe, 21 inches long, Mark No. 2CCP-PSSP047, was not signed or dated, when the appropriate blocks of the Bill of Material had been filled in with a Ledger Control Number.

A QC note on Shop Fabrication Sheet No. 317005, Revision 0, sheet 2 of 4 for hanger Code Serial No. 5280-4095, Mark No. 1E61-H005 states, " Stamp Heat Number from Stock on to Item K."
Contrary to the above requirement, the heat number had not been stamped on the portion (1/2 of item K), which had been welded to item E of the hanger.

Shop Fabrication Sheet 3-082-071-027-B, " Detail
'E' for L.H. Safety Inj R.C. Support," requires 1/2 inch minimum fillet weld.
Contrary to the above, fillet welds D and E were measured with a fillet weld gauge and were found to be approximately 1/16 inch under the required 1/2 inch minimwn.
D.
Stone and Webster Specification Nc. 2BVS-59, paragraph 28.28 states in part, "The seller shall visually examine all welds...."

Docket No. 99900701/81 Contrary to the above, ~it could 'not be ascertained that the seller, Power Piping Company, had visually examined all welds, in that shop fabrication sheet No. 3-082-071-027 did not require or have provisions-for visual examination of completed welds.

NA-4451 in Section III of-the ASME Code states in part, "...' Measures shall be established to assure that processes, including welding, and heat treating are controlled -in accordance with the rules of this1 Section and are accomplished by qualified personnel using qualified procedures.
Contrary to. the above, it could not be ascertained from records that welding on shop fabrication sheet No. 3-082-071-027 had been accomplished-by qualified personnel using qualified procedures, in thatLthe welder or
'.the welding procedures specification used could not be identified.
