ASSURING CHANNEL INDEPENDENCE IN ENGINEERED SAFETY FEATURES ACTU Supporting Statement for Requesting GMB Approval Under the Paperwork Reduction Act of 1980 (P.L.96-511) i 1.
Justification The background section of IE hullettn 81, describes a spurious ESFAS that occurred at the Davis-Besse facility, and the potential for adversely affecting the ESFAS by interconnections between redundant ESFAS channels.
On the one hand, these interconnections can lead to failures of comission-(i.e., st iriously actuating the ESFAS when such actuation is not required);
or, on the other hand, they can lead to failures of commission. (i.e., failure to actuate the ESFAS when such actuation is required to mitigate the effects of operational transients or accidents).
The primary purpose of this information collection is ta assure the independence of redundant ESFAS channels through a program of reviews, analyses and tests, and ultimately by the implementation of changes, if needed, by licensees and certain holders of construction pemits.
The reporting requirements are needed to ensure that this program is conducted in a timely manner and to identify additional problem areas.
The infomation

obtained will be reviewed by regional and headquarters personnel to verify l
completeness and to determine the need for additional or supplemental tests and analyses.
Since the requested reviews and analyses pertaining to channel independence have not been perfomed to date, data concerning the independence of redundant ESFAS channels is not presently available from any source.

Description of Survey Plan This requirement will be applicable to all operating nuclear power plants l
l (69) and those olants under construction (17) which are expecting to receive

the Office of Nuclear Reactor Regulation.

Time Schedule for Data Collection and Publication The licensees and construction permit holders are requested to respond within 90 days of receipt of the information request. NRC will immediately i
begin review and evaluation of the responses and expects to have all completed'within 6 months.

Consultations Outside the Agency Concurrent with this request for OMB clearence, we are requesting comments 1
and estimates of industry impact from the Atomic Industrial Forum.
This information should be available in mid-June 1981 and will be forwarded to you at that time.

ASSURINE CHANNEL INDEPENDENCE IN ENGINEERED SAFETY FEATURES ACTUATION SYSTEMS (ESFAS)
Purpose:
The intent of this bulletin is to improve nuclear power plant safety by assuring the independence of redundant channels of engineered safety features actuation systems (ESFAS).
Channels that provide signals for the same protective function should be independent and physically separated to accomplish decoucLing of the effects of un-safe environmental factors, electrical transients, and physical accident consequen-ces docbmented in the design bases, and to reduce the Li,kelihood of interactions between channels during maintenance coerations or in th'e event of channel malfune-tiens.
Toward this end, this bulletin requests Licensees and holders of construc-tion permits for nuclear power facilities to:
(a) review their ESFAS designs for interconnections between redundant channels; (b) es4ivate the potential effects of each interconnection; and (c) provide informa; ion on this matter to the NRC.

The above concern was brought to light by an event that o-at Davis-9 esse (Docket No. 50-346) on December 5,1980.

As described later, the redundant instrument channels of the

IEB 81-May,1981 Page 2 of 5 Davis-Gesse safety features actuation system (SFAS) are interconnected electrically in a manner that compromises channel independence. The Davis-Sesse SFA$ was design-ed and supplied by Consolidated Controls Corporation and 9echtel Corporation.
On December 1,1980, four days prior to the inadvertent SFAS actuation, the status lignts for SFAS Channels 1 and 3 started blinking synchronously between the tripped j
and untripped states. On Decemcer 5,1980, the SFAS was inadvertently actuated I
while instrument control personnel were troubleshooting for grounds in the SFAS Channel 3 cabinet.
This actuation started the emergency core cooling system (ECCS) pumps and immediately transferred them to the recirculatien mode of operation (i.e.,
their suction alignment was transferred from the borated water storage tank to the emergency sump).
Since the plant was in a hot shutdown mode, this event did not endanger the health and safety of the public. However, if prompt operator action had not been taken,- the transferring of the ECCS pumps' suction to a dry sumo could have led to pump damage and subsequently to the loss of an ECCS function.
A review of the Davis-Sesse SFAS wiring diagrams revealed that the.115 volts direct l
current' (115V del.and 24V de power supplies are designed' to include floating returns.
In addition, the floating returns for the 115V de and 24V de power supplies ass'ociat-ed with SFAS Channels 1 and 3 are interconnected electrically (hardwired) by design
~
so as to form common power supply returns.
(Similar interconnections exist between SFAS Channels 2 and 4.)
In the Davis-Sesse SFAS design, the +15V de power sucolies provide power to the instrument / sensing channels, and the 24V de power supplies pro-vide power to the actuation / Logic channels.
The investigation of this event revealed that an undetected electrical fault had existed within the y,15V de power supply of SFAS Channel 1 (PS-07) for fou'r days prior

to the SFAS actuation.
This electrical fault apparently placed a potential, of 120 volts alternating current (120V ac), with reference to chassis ground, on the inter-connected power supoly returns and components associated with SFAS Channels 1 and 3.

The effects of this sustained potential on components associated with SFAS Channels 1 and 3 are not completely known; however, the Licensee has determined that this pe-tential caused the setpoints of the bistables of Channels-1 and 3 to deviate from s

their original settings.
T..us, a failure within an SFAS instrument channel at the Davis-Sesse facility could lead to one of two types of systematje failures:
(a)
Tyoe I, (i.e., f ailures of commission that would initiate spurious SFAS actuation when such actuation is not requi red); or (b) Tyce II, (i.e., f ailures of omission that would inhibit SFAS actuation when such actuation is required to mitigate the effects of transients or accidents). Based on the above, it appears that the common power supply returns used at Davis-9 esse provide a path whereby a single failure can adversely affect nore than one SFAS channel.
The Licensee, in Licensee Event Report 80488, attributed the SFAS, actuation to a
" component failure".
A more complete assessment would also include the effects of the interconnections of the de power supply returns. These interconnections con-tributed to the event by providing a path for a fault in ' ole channel to cause the i
compt.ete loss of a power supply in another channel. As stated previously, an un-l detected f ault in the.1,15V de power supply of SFAS Channel,1 (PS-07) plac'ed a u"
120V ac potential, with refe'rence to chassis ground, on the commen power suoply s
returns of SFAS Channels t and 3.
This 120V ac potential subsecuen Ly led to the a

complete failure of PS-07 when the power supply common of Channel 3 was shorted:te chassis ground.
This short occurred when a lamp'was being removed from an,oGrpyt~

module in SFAS Channel 3.
Since the Davis-Sesse SFAS bistables are designed to trip on loss of channel power, the SFAS bistables in Channel 1 tripped when PS-07 failed. Prior,tc the tripping of the SFAS bistables in Channel 1, some of the' SFAS bista61.es in Channel 3 were also in a trioped posi ion.
Therefore, since the t

These tasts and analyses should place special emchasis on the potential for adversely affecting the setpoints of bistable devices by electrical failures or transients.
Holders of operating Licenses and holders of construction permits for nuclear power facilities expecting to receive an operating License by January 1983 should submit the above recuested information within ninety (90) days of receipt of this bulletin.
ALL other holders of construction permits should consider this document as an infor-mation notice for which no written response is required.
