SUBJECT:
BOONE COUNTY HOSPITAL, COLUMBIA, MO 65201 LICENSE NO. 24-01565-01 As a result of a violation of a DOI regulation, the State of Washington suspended the subject licensee's weste burial permit indefinitely. The Severity Level of the violation is III.
In accordance with the guidar.ca set forth in EGM-81-14A, this case requires the lessee of a Notice of Violation by Region III.
Attached for your information are copies of the letter to the licensee, and the Notice of Violation.
