V APPENDIX A NOTICE OF VIOLATION frortheast Nuclear Energy Company Millstone Unit 1 Hartford, Connecticut 06101 License No. DPR-21 Docket No. 50-245 As a result of the inspection conductec usi July 12 through September 26, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified:
A.
Technical Specification 3.6.A.4 states, "With the mode switch in the startup/ hot standby or run mode, operation without forced circulation shall not be permitted".
Contrary to the above, on September 15,1981, at 1040 hours0.012 days <br />0.289 hours <br />0.00172 weeks <br />3.9572e-4 months <br />, the reactor was operated in the startup/ hot standby mode without forced circulation for 17 minutes. At 1057 hours0.0122 days <br />0.294 hours <br />0.00175 weeks <br />4.021885e-4 months <br />, the reactor was shutdown by a trip of the Reactor Protection System.
This is a Severity Level IV Violation (Supplement I).
B.
Technical Specification 6.8.1 statec in part:
" Written procedures shall be established, implemented and maintained covering the activities referenced below; a.
The application procedures recomended in Appendix A of Regulatory Guide 1.33, November 1972..."
Regulatory Guide 1.33 dated November 3,1972, Appendix A, paragraph G requires, " Procedures for Control of Radioactivity (for limiting materials released to environment and limiting personnel exposure),1. Liquid Radioactive Waste System, a. Callection, Demineralization, Filtering, Evaporating and Concentrating, and Neutralizing... "
Regulatory Guide 1.33 dated November 3,1972, Appendix A, paragraph I, states ir, part, " Procedures for Performing Maintenance 1. Maintenance which can a'fect the perfomance of safety-related equipment should be properly preplanned and performed in accordance with written procedures, docariented instructions, or drawings appropriate to the circumstances".

Contrary to the above, on September 24, 1981, an operator attempted to conduct a blowdown of the "B" radioactive liquid waste concentrator using an air hose connected to a steam source. Neither did Station Operating Procedure 312 C revision 7 dated 12-19-77, " Liquid Radwaste (Concentrator Operation)" nor any station maintenance procedure address this technique of blowdown. As a consequence, the hose -
separated spewing. radioactive material and steam-water mixture onto the operator.
This is a Severity Level IV Violation (Supplement I).
C.
Unit 1 Technical Specificatioa 6.11 states, " Procedures for personnel raciation protection shall be prepared consistent with the requirements of 10 CFR part 20 and shall be approved, maintained and adhered to for all operations involving personnel radiation exposure".
Stan fardized Health Physics Procedure 4912 Revision 2 dated May 12, 1981, section 3, defines areas requiring Radiation Work Pennits (RWP) stating, in part, "3.
Definitions 3.1 RWP Areas 3.1.1 High radiation areas, contaminated areas, airborne radioactivit.v areas... any areas posted with an
' RWP requi red' sign,... "
Standardized Health Physics Procedure 4912 Revision 2 dated May 12, 1981, section 8.1.9 states, " Personnel may be issued a blanket RWP for routine work with the HP Supervisor approval. Specific jobs or work identified during surveillance tours and inspections shall not be done on the Blanket RWP. Separate RWP's shall be initiated for such work."
Contrary to the above, at about 2130 on September 24, *^81, a plant operator entered a radiological area to attempt to clear a blockage in piping associated with the "B" radioactive liquid waste concentrator while authorized entrance on RWP No. 114962 marked "0PS BLNKT" and "For routine plant rounds and equipiaent operation and surveillance."
The area entered was posted "RWP required for entry" and had been indica-ted as containing loose surface contamination levels of 5000 DPM per 100 square centimeters on a survey conducted at about 1315 on September 24, 1981.
This is a Severity Level V Violation (Supplement IV).

Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is.hereby required to submit to this office within twenty-five days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be' achieved. Under the authority of Section 182'of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
The responses dieected by this Notice are not subject to the clearance procedures of.the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
