Bris letter r!cscribes a reportable occurrence involv-ing a minimum flow valve in t% Safety Injection System.

Submittal is in accordance with the reporting requirements stipulated in Section G.9.2 of Appendix A to the Provisional Operati ng License DPR-13.

In con.iunction with the testino of the safety injection hydraulic valves

<,everal cycles of safety injection initiation were performed.

ifhile in mode 3 iluving the final llot Safety In.icction System Test CV-36, a valve in the recirculation line to the condenser, failed to close resulting in 15,000 collons ef refueling water beinq pumped into the condenser.

1 As a result the volume of water in the refueling water storage tank fell below l

t.cchnical specification requirements.

Volume was restored within 2 hours2.314815e-5 days <br />5.555556e-4 hours <br />3.306878e-6 weeks <br />7.61e-7 months <br />.

I liad this f ailure occurred during an actual LOCA the delivery of 220,000 qallons of Snrated water required by the safety analysis would not have occurred beca'ase of this leakage path.

The valve was inspected and a c ontroller in the actuator was identified as the cause of the problem.

This

< ontroller will be repaired or replaced prior to startup.

A complete report on this matter together with a t.icensee Fvent Report will be foe.varded within two weeks.
