APPENDIX A NOTICE OF VIOLATION Westinghouse Electric Corporation Docket No. 70-1151 Columbia Fuel Plant License No. SNM-1107 As a result of the inspection conducted on August 10-14, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.
A.
License Condition Number 9 requires that licensed material be used in accordance with statements, representations and conditions contained in Sections 2, 3 and 4 of the licensee's application dated December 30, 1976, and supplements thereto.
Section 3.1.2 states that the shift supervisor will assure that all operations under his control are carried out in accordance with the schedules, processing procedures, nuclear criticality safety and radiation protection limits, and other instructions supplied by higher management.
Contrary to the above, the requirement that individual workers survey themselves for contamination when exiting the control area was not met in that at least four individuals were observed leaving the control area via the change room on August 10, 1981, without checking themselves for contami-nation.
This is a Severity Level V Violation (Supplement IV.E).
B.
License Condition Number 38 requires that the licensee shall perform tests, measurements, or surveys as may be necessary for 'im to comply with action points and limits in this license.
i Contrary to the above, the license requirement that surveys (operational and l
functional checks of survey instruments in use) be performed to assure the operability of survey instruments was not met in that:
1.
The facility housekeeping daily check sheet procedure did not contain l
the required operational or functional checks on survey instruments.
l 2.
Survey checks had not been performed on survey instruments in use for at Ic t four days.
3.
Inoperable instruments were observed by the inspector on August 10 and l
11, 1981.

Section 3.2.4 and Table 3.2.4.1 states that immediate decontamination or area isolation will be required if the average contamination level in an area is greater than five times '.he appropriate action level.
The action level for fixed (total) contamination in Table 3.2.4.1 for a clean area is 250 cpm measured sith a PAC-4G instrument or equivalent.
Contrary to the above, the requirement for immediate decontamination or area isolation was not met in that no action was tak-n by the licensee when fixed contamination levels in excess of 1250 cpm with a PAC-4G survey instrument were detected in the clean side of the change room on August 10, 1981.
This is a Severity Level VI Violation (Supplement IV.F).
D.
License Condition Number 38 requires that the licensee shall perform teG.

Contrary to the above, the license requirement for performing surveys for fixed contamination was not met in that surveys for fixed contamination had not been performed by the licensee.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including: (1) admission or denial of the alleged viola-tions; (2) the reasons for the violations if admitted; (3) the corrective steps which have been taken and the results achieved; (4) corrective steps which will i
be taken to avoid further violations; and (5) the date when full compliance will l
be achieved. Consideration may be given to extending your response time fer good l
cause shown.
Under the authority of Section 182 of the Atomic Energy Act of j
1954, as amended, this response shall be submitted under oath or affirmation.
