This letter, telecopied to your office, is intended to satisfy the requirement for prompt notification of a Reportable Occurrence in accordance with Specification 6.11.2.a of the Duane Arnold Energy Center Technical Specifications.
Technical Specification paragraph (s) violated:
3.7.E.1 Description Occurrence:
During a valve line up check on 10/18/81, "D" main steam line MSIV leakage control system manual valve V-84-16 was found closed. The correct position for this valve is locked open. This misalignment affectively made the leakage control system for the "D" main steam line MSIVs inoperable. The cause is under investigation.
