Dear Mr. Stello:

1 Pursuant to the provisions of 10 CFR 2.201 and your letter of September 2, 1981, tSe following infomation is submitted regarding the July 29, 1981 occurrence at Nine Mile Point Unit 1.

This occurrence involved placing both emergency cooling system isolation logic keylock switches in the bypass condition, which, in turn, defeated both the high radiation and steamline high flow isolation functions. Niagara Mohawk concurs with the findings contained in the Notice of Violation.

Accordingly, enclosed is this Corporation's check made payable to the Treasurer of the United States of America (10 CFR 2.205) in the amount of $50,000.

This Technical Specification violation resulted from failure to follcw administrative procedures. The following corrective action was taken subsequent to two prior Technical Specification violations:

1)

The equipment status log now reautres the inclusion and verification of the applicable Technical Specification references.

2)

The jumper / block log has been revised to include applicable Technical Specification requirements.

In addition, the log sheets have been revised to include Station Shif t Supervisor and Chief Shift Operator pernission to install or remove jumper / blocks and Station Shift Supervisor ecknowledgment that jumper / blocks have been installed.

The following corrective actions have been implemented, since the July 29, 1981 occurrence:

1)

The event has b2en reviewed and discussed with all personnel involved.

2)

A memorandum has been issued to all operating personnel reminding them of the importance of Technical Specification requirements when removing safety equipment from service.

3)

The emergency cooling operating procedure has been revised to better clarify the purpose of the bypass switches.

As a result of these actions, all shif t personnel have become better aware of 1e need to consult the Technical Specifications in the course of daily operations.

In addition, management has become aware of the increased need for training in the area of site administrative procedures and their relevance in daily operction.

In order to prevent recurrence of this type of event, managenent has taken the following steps to provide the necessary element of motivation to site personnel:

1)

A revised training program has been implemented to stress all aspects of the Technical Specifications and Administrative Procedures with respect to daily operations.

2)

A Technical Specification cross reference to facilitate review of all Technical Specifications applicable to a given component will be developed and implenented by Decenber 31, 1981.

3)

Managenent meetings will be held with each shift during the nomal training cycle to discuss personal interactions required when removing a safety related component from service and also the interactions of other departments with the Operations Department.

This will be implemented by December 31, 1981.
