APPENDIX A NOTICE OF VIOLATION Northeast Nuclear Energy Company Docket No:
50-245 Hartford, Connecticut 06101 License No. DPR-21 As a result of the inspection conducted on August 10-14, 1981 and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1930), the I
following violation was identified:
10 CFR 50, Appendix B, Criterion XI states, in part, that:
... all testing
... is... performed in accordance with written procedures which incorporate the requircments and acceptance limits contained i.' applicable design documents.
... Test results shall be documented and evaluated to assure that test require-ments have been satisfied."
Contrary to the above, test data were not recorded in accordance with pro-cedures; acceptance criteria and limits were not specified in test procedures; test procedures contained insufficient information to conduct the tests and to determine that the testing activities had been satifactot ily accomplished; corrective actions taken were not identif'ed in the test records; and test results were not documented and properly reviewed, as determined by the follow-ing examples:
a.
Average control rod scram insertion times were not calculated and not recorded, as required by RE Form 1051-3, on April 19, 1981.
b.
The formula in step 7.4.5 of Operations Form 690B-1 for shutdown margin (SDM) calculation was incorrect; SDM value calculated on April 17, 1981 was not correct.
Information and references in the procedures SP 6905 and 6908-1 were not sufficient to perform the measurements. The inspector verified that SDM met Technical Specifications.
c.
Reactor period was not recorded in the data sheet for a test performed June I
29, 1981 as required by procedure SP 1050 and Form 1050-1. The procedure did not provide acceptance criteria, references, and sufficient data reduc-tion details.
d.
Reactor Engineering Procedure RE 1057 specified neither acceptance criteria nor references.

The on-demand program outputs, 0D-10, Option 24 for June 26, 1981 were not documented in test records as required by proceoure SP 1041.
Incorrect values were recorded in RE data. form 1041-1 for the test performed June 26, 1981.
Reviewers failed to recognize the errors.

Acceptance criteria were not provided in Intermediate Range Monitor Calibration and the corrective calibration performed April 27, 1981 was not identified in the original test packages in which the deficiency was identified.
These examples of noncompliance in the Reactor Engineering test program are collectively considered a Severity Level IV Violation (Supplement I).
Pursuant to the provisions of 10 CFR 2.201, Northeast Nuclear Energy Company is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1354, as amended, this response shall be submitted under oath or affirmation.
Where good cause is shown, consideration will be given to extending your response time.
The responses directed by this Notice are not subject to the clearance pro-cedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
