NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on January 19-23, 1981, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:

A.

' Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality be accomplished in accordance with docurented instructions, procedures, or drawings, of a type appropriate to the circumstances.

The following requirements are included in the various hot solder pot instructions and test procedures:

Paragraph 4.0.1, of In-Process Inspection Procedure No. QC-146, Revision 12, dated September 1980 states, " Solder Pot 500 to 600 F."

2.

Paragraphs 1.f and 2. of Testing Procedure, undated, indicates a solder pot test will be conducted with results being recorded in the log book.

Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality be accomplished in accordance with documented instructions, procedures, or drawings, of a type appropriate to the circumstances.

An unnumbe, red',, unsigned, and undated procedure entitled, " Proc (Jure For Repair Request" was provided to the NRC inspector as the document used to process the appropriate forms (Repair Request and Respool Request) and control nonconforming material The procedure indicates applicability to all reels of questionable material beld for repair.

The procedure states in part, "Under no circumstances is the material to be moved to the repair area without

2 "The responsible Foreman shall fill out the following information on the Repair Request Form in triplicate... THE FOREMAN'S SIGNATURE AND AREA SUPERINTENDENf'S SIGNATURE (sic)."

Contrary to the above, material had been moved to the interlock at mor repair area but the foreman had not signed the following Repair Request Forms (among others) for reels of nonconforming material located in the interlock armor repair area:

(a) Form No. 02057 for Part No. T-7732 4/16, Order No. AE-85118-3, Reel No. 4197, and (b) Form No. 08560 for Part No.

T-7420 61/16, Order No. E-88341-1, Reel No. 1017.

The NRC inspector observed that the interlock armor of item (a) was being repaired.

Criterion VI of Appendix B to 10 CFR Part 50 requires that measures be established to control the issuance of instructions, procedures, and drawings, and related changes, which prcscribe all activities affecting quality.

It is also required that these measures assure that the aforementioned documents and related changes, be reviewed for adequacy, approved for release by authorized personnel, distributed to and used at the location where the prescribed activity is performed.

Manufacturing Procedure No. 030, Revision 0, dated April 1978, states in part, "At each extruder there are Extruding Guideline Books by tab number.

Each tab number product should have a guideline.

If Extruding Guideline is not available, inform your foreran who will have one made out.

The Extruding Guidelines are used as guidelines for setting up the extruder and for running speeds."

Contrary to the above, the established measures did not assure that Primary Extrusion Operating Guidelines are approved for release by authorized personnel.

These documents contain a space for an approval signature; however, numerous documents located at the extruding machines did not exhibit an approval signature.
