~O APPENDIX A NOTICE OF VIOLATION Kansas Gas and Electric Company Docket:

STN 50-482/81-16 Wolf Creek CPPR-147 As a result of inspection activity conducted during the month of September 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violation was identified.

10 CFR Part 50, Appendix B, requires in Criterion V that activities affecting quality shall be prescribed by documented instructions, procedures, or drawings of a type appropriate to the circumstances and shall be accomplished in accordance with these inspections, procedures, or drawings.

The SNUPPS Quality Assurance Program for Design and Construction manual requires in paragraph 178.1.13 that preservation of appropriate materials and equipment is controlled in accordance with written procedures, and that Daniel International Corporation (Daniel) has primary responsibility for handling, storing, and preserving material delivered to the plant site.

Daniel requires, in Work Procedure WP-XI-302, Revision 2, paragraph 1.2, that requirements are intended to assure that the quality of items is not degraded as a result of storage practices.

Additionally, ANSI Standard N45.2.3 is involved by reference in procedures QCP-I-01, Revision 10, September 9, 1980.

ANSI N45.2.3-1973, Housekeeping during the Construction Phase of Nuclear Power Plants, requires in paragrpah 3.3 that materials and equipment delivered to the work area shall be placed so that they are accessible but do not hinder construction progress.

However, material and equip-ment shall be so positioned that it will not be damaged by construction e

l activity.

Contrary to the above, on three separate occasions the main feeder breaker for either NB01 or NB02 4160 Volt Class lE switchgear was sbserved removed l

from its own cubicle and left unattended and unprotected in the work-way

adjacent to the switchgear locat d at elevation 2000' of the Control Build-ing.

Breaker NB0112 was obser/ed on August 12, 1981.

This occurrame was s-discussed with the KG&E QA Manager, Wolf Creek Site, during an exit debriefing on August 13, 1981.

Breaker NB0212 was observed on September 9, l

1981.

This occurrence was discussed with a KG&E Construction Engineer,

Breaker NB0212 was observed again on September 10, 1981.

An adequate procedure, specific to the protection of removed breakers has not been provided as required and safety-related electrical breakers, whose failure would remove one oc both sets of 4160 Volt Class lE switch-gear as an essent?al service power source, were not protected from possible damage by construction activity.

This is a Severity Level IV violation.

Pursuant to the provisions of 10 CFR 2.201, Kansas Gas and Electric Company is hereby requrio2 to submit to this office within 30 days of the date of this Notice, a written statement or explanation in reply, including:

(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which wili t'e taken to avoid further items of nonco;npliance; and (3) tna date when full compliance will be achieved.

Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.

Consideration may be given to extending your response time for good cause shown.

The responses directed by this Notice are not subject to the clearance procedures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
