2' In accordance with Plant Technical Specification 6.9.2.a.2, the following event is reported.
At 1542 hours0.0178 days <br />0.428 hours <br />0.00255 weeks <br />5.86731e-4 months <br /> on September 25, 1981, while the Reactor Operator was re-aligning Group 2, Bank D control rods with Group 1, Bank D, he noted that the Rod Position Indicators showed no movement for Group 2, Bank D control rods.
Instrumentation and Control personnel investigating the problem disconnected the Lift Coil Disconnect Switches to Group 1, Bank D control rods and an attempt was made to move Group 2, Bank D rods.
No movement was observed on the Rod Position Indicators.
In accordance with the Plant Operating Manual, Volume 13, Emergency Action Levels, Item 3.2, an Unusual Event was declared at 1650 hours0.0191 days <br />0.458 hours <br />0.00273 weeks <br />6.27825e-4 months <br />, and a reduc-tion in power to hot shutdown was initiated at 1658 hours0.0192 days <br />0.461 hours <br />0.00274 weeks <br />6.30869e-4 months <br />.
During manual l
rod insertion, the Reactor Operator noted that there was no movement in l
Group 2, Bank B and Group 2, Shutdown Bank B.
The reactor was tripped at 1828 houre and all control rods inserted except L-ll which is inoperable (Plant Modificatica #569). The Unusual Event was also terminated at this time.
Instrumentation and Control personnel determined the cause of failure to be'a faulty input / output amplifier card in Rod Control Power Cabinet 2BD.
The faulty amplifier card was replaced and control rod operability was
-6N demonstrated in accordance with Periodic Test 21.2 at 2020 hcurs on M y September 25, 1981. Throughout this event the subject control rods were always capable of responding to a reactor trip so there was no threat to I
the public health and safety.
