F-APPENDLt A riOTICE OF JIOLATI0tl Public Service Electric and Gas Company Docket No. 50-354 Hope Creek Unit 1 License No. CPPR-120 As a result of the inspection conducted on August 3-3', 1981, and in accordance with the Interim Enforcement Folicy, 45 FR 66754 (0ctuoer 7, 1980), the following violation was identified:
10 CFR 50, Appendix B, Criterion II states, in part, that: "The quality assurance program shall provide for indoctrination and training of personnel performing acti,lties affecting quality as necessary tc assure that suiteble proficiency is achieved and maintained."
Section 16.4.2 of Chapter 16 of the PSAR states, in part, that:
" Personnel assigned to projects are provided with specific indoctrination and training covering the project procedures applicable to their work. This is accom-plished by general discussion of specific procedures and -dividual training by project supervisisn and staff specialists."
Paragraph 7.4 of Bechtel Specific Work Plan / Procedure SWP/P-54. Installation of Process Pipe Systems, states, in part, that:
"The respor:sible Field Engineer and the Piping Superintendent shall ensure that the requisites for conmencing work, and the controls during the work, are adhered to..."
Centrary to the above, as of August 12, 1981, it was determined that pipe-fitters were not aware of the jobsite litaitations controlling cold pulling of pipe because their inrnediate foreman did not explain these limitations to the personnel under his supervision.
This is a Severity Level V Violation (Supplement II).
Pursuant to the pitiisions of 10 CFR 2.201, Public Service Electric and Gas Com-pany is hereby required to submit to this office within thirty days of the date of this Notice, a written statement or explanation in reply, including:
(1)the corrective steps which have been taken and tne results achieved; (2) corrective sttes which will be taken to avoid further violations; and, (3) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation. Where good cause is shown, consideration will be given to axtending your response time.
The responses directed by this Notice are not subject to the clearance procedures of the Office of Management.and Budget as required by the Papenvork Reduction' Act of 1980, PL 96-511.
