/A This is to infonn the Commission that a Notice of Violation and a N k%

Proposed Imposition of Civil Penalties in the amount of Forty Thousand Dollars will be issJed on or about September 24ci981 to the Tennessee Valley Authority, Chattanooga, TN. This action is based on apparent items of noncompliance at the Sequoyah facility which resulted in exceeding a Technical Specification Limitir.g Condition for Operation. The immediate cause of the event was a failure to return three recirculation valves in the containment spray system to their normally locked-shut position, as required by procedure. This failure rendered a system designed to prevent or mitigate a serious safety event unable to perform its intended function.

A news release has been prepared and will be issued about the time the licensee receives the Notice. The State of Tennessee will be notified.

It should be noted that the licensee has not been specifica~ly informed of the enforcement action. The schedule of issuance and notification is:
