Inspection on August 12-13, 1981 (Report No. 50-454/81-12; 50-455,'80-10)
Areas Inspected:
Licensee's corrective action relative to previo2s inspec-
tion findings. Followup items reported pursuant to 10 CFR 50.55(e). Followup
Inspection and Enforcement Bulletin action. Reactor Protective System instru-
mentation work activities and records. The inspection involved a total of
12 inspector-hours onsite by one NRC inspector including 0 inspector-hours
onsite during offshifts.
Results: Of the four areas inspected, no items of neacompliance were identi-
fied in three areas. One item of noncompliance was identified in one area
(violation-protection and preservation of equipment).

Identification of instrument
lines by colored tags. Licensee has devised a calor coding system for
identifying redundant instrument lines during installation to assist
in the verification that the separation criteria is maintained. The
lines will be color coded with multi-colored tapes until permanent
metal tags can be attached. This item is closed.
50-454/79-18-01 (Closed) Noncompliance. Failure to document corrective
actions on controlled documents. The nonconformance and corrective
actions have been properly documented. Procedures are in place re-
quiring contractor to initiate nonconformance reports for nonconform-
ances identified on vendor supplied items requiring corrective actions.

Inspection and Enforcement Bulletins
" Loss of Charcoal from Standard Type II, 2-inch
Tray Absorber Cells." Filters to be used at Byron
have been examined and tested by the vendor, Farr
Filter Corperation. The vendor's examination and
test revealed no deficiencies in the rivet spacing
or screet/ casing separation. This bulletin is closed.
" Vacuum Condition Resulting in Damage to Chemical
Volume Control System (CVCS) Hoidup Tanks." The
l
tanks are designed for an external pressure. A
nitrogen cover blanket is supplied by a header.
High and low pressures in the header are annunciated
locally and alarmed on the main control board.
In
case of rapid removal of liquid from the tank, a
cover gas makeup path sized to admit gas at the maximum
l
liquid removal rate is provided. This Bulletin is
closed.
Flow Blockage of Cooling Water to Safety System
,
'
Components by Corbicule Sp. (Asiatic Clam) and
Mytilus Sp. (Mussel). Asiatic clams have not been
l
found in the Rock River, the source of cooling water
l
for Byron, and should not cause cooling water flow
blockage to safety systems. This bulletin is closed.

Items Reported Pursuant to 10 CFR 50.55(e)

Incorrect weights for Velan globe valves identified when compared to
i
drawings. Correct weights have been determined by recalculation and/
or actual weighing by Velan Valbe Corporation and the affected drawings
l
have been corrected and redistributed. This item is closed.

Instrumentation

The inspector observed the installation of pressure and flow instru-
mentation associated with the reactor protective system and examined
installation and inspection documentation relative to those instruments
for which installation activities are complete.
The inspector noted that instruments had been calibrated and installed
on their respective racks. The instruments on rack IPL 75J were being
l
used as storage hangers for coiled welding hoses, welding face shields,
hats, grinding masks, and as storage racks for scrap pipe.
Hoses and other welder's equipment were observed on rack IPL 52J.
This
failure to provide measures for the protection and preservation of safety
related equipment is considered an item of noncompliance with 10 CFR 30,
Appendix B, Criterion XIII as described in Appendix A of the report
transmittal letter.
