Joseph M. Farley Nuclear Plant NRC Inspection of May 13 - June 15,1981
Dear Mr. Kellogg:
This letter refers to the following violation in the subject inspection report:
" Technical Specification 6.8 requires that written procedures shall be established, implemented, and maintained. Administrative Pro-cedure No. 16, " Conduct of Operation - Operations Group", requires the shift turnover of significant equipment deficiencies.
Contrary to the above, the Shift Turnover procedure was not followed as significant equipment deficiencies were not turned over in that:
on May 20, 1981, the Unit 1 Core Subcooling Monitor had one channel removed for modification and neither the Shift Supervisor nor the Control Room Operator was aware of the status of this equipment; and on June 5,1981, a Unit 2 reactor trip occurred with one control rod indicating that it had not fully inserted, this condition had been diagnosed on June 3 as one train of the digital rod position s
indicator system malfunctioning which was not turned over to the following shifts."
Response
With regard to the referenced violation, the following corrective action has been or is being accomplished.
1.
The individuals involved in this incident have been counselad with on the significance of proper shift turnover.

During Operator requalification training, the importance of insuring all information is turned over der'nu shift turnover will continue to be stressed.
3.
Operation supervision will continue to perform spot checks of shift turnover sheets for review of turnover content.
4.
An Operations Memorandum has been written to all lic6nsed per-sonnel addressing this incident and its significance.
