APPENDIX A NOTICE OF VIOLATION Tennessee Valley Authority Docket No. 50-553 Phipps Bend 1 License No. CPPR-162 As a result of the inspection conducted on May 5-8, 1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7, 1980), the following violations were identified.

A.

10 CFR 50, Appendix B, Criterion V, as implemented by Phipps Bend PSAR Section 17.1A.5, requires that activities affecting quality be prescribed and accomplished by appropriate documented procedures, instructions and drawings. TVA procedure CEP 15.03 requires identification of questionable conditions of compliance on a QC Investigation Report (QCIR). TVA instruc-tion QCI W-116R0 requires a chart of the location and size of prepared cavities for base metal repairs.

Contrary to the above, on May 6,1981, proper instructions had not been provided for repairs to Containment Sump Liner T13-XX 004-1 and irstructions for documenting repair cavities and questionable conditions had not been met.

The sketch provided with the instructions for completed base metal repairs (as described in QCIR 21617) did not adequately identify the areas to be repaired on sump T13-XX 004-1. Some areas of excessive undercut and a gouge (questionable conditions of compliance) on the sump were not identi-fied on a QCIR. A chart had not been prepared giving the location and depth of weld repair cavities on the sump.

This is a Severity Level V Violation (Supplement II.E).

B.

10 CFR 50, Appendix B, Criterion IX, as implemented by Phipps Bend PSAR Section 17.1A.9, requires that measures be established to assure that special processes are controlled and accomplished using procedures ' s, accordance with applicable codes, standards, special criteria, etc.

Containment Anchor Bolt Chair Welds P1 T2302710 and 714 require 5/8 inch fillet welds as specified on drawing F0109A.

2.

Procedure QCI N-501 R4 requires recording of material thickness or weld size on the process control cards. This information may be used by a QC inspector in verifying proper material thickness and/or weld size.

The cards for the welds described above recorded a thickness of.422 inches. The cards for safety related piping welds (Air Positive Seal System) P1 P6100038 and 39 recorded a.133 thickness.

Contrary to the above, on May 6,1981, the special oncess of fillet weld inspection was not adequately controlled or accomplished in that:

1.

When checked with a gage the 5/8 inch fillet welds identified P1 T2302710 and 714, which had been accepted, were undersized.

2.

Thicknesses specified on process control cards for welds P1 T2302710 i

and 714 and welds P1 P6100038 and 39 represented neither the material j

thickness nor the weld size.

This is a Severity Level V Violation (Supplement II.E).

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within thirty days of the date of this Notice, a written statemen.t or explanation in reply, including:

(1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the correttive steps which have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved.

Consideration may be given to extending your response time for good cause shown. Under the authority of Secticn 182 of the Atomic Energy Act of 1954, as amended, this response shall be ubmitted under oath or affirmation.
