R Appendix A NOTICE OF VIOLATION Commonwealth Edison Company Docket No. 50-254 As a result of the inspection conducted on July 1, 1981, through August 4, 1981, and in accordance with the Interim Enforcemsnt Policy, 45 FR 66754 (October 7,1980), the following violations were identified:

10 CFR 50.72 states, "Each licensee of a nuclear power reactor licensed under 50.21 or 50.22 shall notify the NRC Operations Center as soon as possible and in all cases within one hour by telephone of the occurrence of any of the following significant events and shall identify that event as being reported pursuant to this section. Any event resulting in manual or automatic actuation of Engineered Safety Features, including the Reactor Protection System."

Contrary to the above, on July 7,1981, Unit 1 scramed on automatic initiation of the Reactor Protection System. The NRC was not notified of this event within the prescribed time frame.

This is a Severity Level VI violation (Supplement I.F.).

Technical Specifications 6.2.E. states in part, " Radiation cont 31 procedures shall be maintained, made available to all station personnel, and adhered to."

Procedure QRP-18 states, " Danger High Radiation Area signs will be used to mark areas in which a person may receive greater than or equal to 100 mrem /hr. whole body exposure. These areas will always be locked, fenced off, or under direct surveillance to limit access to personnel. These areas will require an 'R' key

and notification of shift engineer for entry."

Contrary to the above, on July 14, 1981, the Unit 1 Refuel Floor High Radiation door was found by the inspector to be unlocked and not under direct surveillance.

This is a Severity Level V violation (Supplement IV.E.2).

The inspection showed that action had been taken to correct the identified items of noncompliance and to prevent recurrence. Consequently, no reply to these items of noncompliance is required and we have no further: questions regarding this matter.
