The condition involves the omission of locking devices on threaded fasteners of certain mechanical shock arrestors as required by the ASME Boiler and Pressure Vessel Code.
Enclosed in accordance with 10CFR50.55(e) are twenty-five copies of a Final Report entitled, " Final Report on the Omission of Locking Devices on the Fasteners of Certain Mech:Inical Shock Arrestors, San Onofre Nuclear Generating Station, Units 2 and 3."

San Onofre Nuclear Generating Station, Units 2 and 3 1
INTRODUCTION i
This report is submitted pursuant to 10CFR50.55(e)(3).
It
{
describes a condition involving the omission of lockwire from threaded fasteners on certain mechanical shock arrestors manufactured by Pacific Scientific, as required by ASME BPV Code.
This condition was discovered during investigation of another condition which was reported to NRC in a letter dated June 1, 1981.
That condition involved use of brass or 4
non-hardened steel washers on fasteners of Certain Mechanical Shock Arrestors manufactured by ITT-Grinnell instead of use of hardened steel washers as required by ASME BPV Code.
I BACKGROUND During a field survey to determine the extent of the use of
)
soft washers in ITT-Grinnell mechanical shock arrestors, it was determined that fasteners on some Pacific Scientific j
supplied mechanical shock arrestors (PS snubbers) had been j
installed without washers or any type locking device.
ASME Boiler and Pressure Vessel Code,Section III, Subsection i
NF requires that, if high strength bolts are used a hardened l
washer is required under the nut or bolt head (paragraph NF-4724).
Other threaded fasteners, except high strength bolts, require a locking device to prevent loosening during E
i service.
The bolts supplied by Pacific Scientific do not require hardened washers, but do require lock wires.
The bolts supplied by ITT-Grinnell require hardened washers.
DISCUSSION j
The following discussion is responsive to 10CFR50.55(e)(3).
Description of Deficiency A Field Change Request to Construction Specification CS-P207 was issued and incorrectly approved to allow field installation I
of both ITT-Grinnell and Pacific Scientific Saubbers without washers.
The instructions for use of lockwire on Pacific Scientific Snubbers is included in the Pacific Scientific installation manual and was overlooked by field construction l.

This condition is reported as a departure from the requirements of the ASME Code.
Corrective Action The schedule for completion of the following corrective actions is under evaluation and development.
That schedule will be furn-ished to the NRC in a supplementary report.
1.
Design Change Package (DCP-5P/S) was issued on July 9,1981.
This DCP provides for:
a)
A change to CS-P207 to eliminate the no-washer option for ITT-Grinnell mechanical shock arrestors and installa-tion of missing washers.
b)
Provides for the replacement of soft washers on safety related ITT-Grinnell sizes 1/4 to 10 with hardened washers.
2.
A Nonconformance Report with supplemental data reports will be written to identify all Pacific Sciantific Snubbers requiring addition of lockwire.
Instructions will be issued from San Onofre Units 2 and 3 project engineering to field construction engineering to install lockwires as required per Pacific Scientific installation manual.
