!!0TICE OF VIOLATION Departaent of the Interior Bureau of Reclamation Yakima Project Office P. O. Cox 1377 Yakima, Washington 98901 License No. 46-17645 01 As a result of the inspection conducted on July 24,1981, and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980),
the following violations were identified:
A.
License Condition 13.A.(1) requires that each sealed source containing licensed material, other than hydrogen-3, with a half-life greater than thirty days and in any form other than gas shall be tested for leakage and/or contamination at intervals not to exceed six months.
Contrary to the above requirement, leak tests of the fifteen 10 millicurie cobalt-60 sources were conducted on fiovember 3,1978, November 19, 1979 and on October 7-21, 1980.
This is a Severity Level V Violation.

B.
License Condition 13.B requires that records of leak test results shall be kept in units of microcuries and maintained for inspection by the Commission.
Contrary to the above reoutrement, at the time of the inspection, records of the results of leak tests conducted on October 7-21, 1980 were not maintained.
This is a Severity Level V Violation.

C.
License Condition 16 s ates that the licensee shall conduct a physical inventory every six months to account for all sealed sources received and possessed under the license.
Contrary to the abo e requirement, sc liannual physical inventories had not been conducted during the period of March 27, 1978 to the date of the inspection.
This is a Severity Level V Violation.

Pursuant to the provisions of 10 CFR 2.201,. Bureau of Reclamation, Yakima Project Office is hereby reouired to submit to this office within thirty days of the date of this flotice, a written statement or explanation in reply, including:
(1) the corrective steps which have been taken and the results achieved; (2) corrective steps which will be taken to avoid further items of noncompliance; and (3) the date when full compliance will be achieved.
Consideration may be given to extending your response time for good cause shown.
The responses directed by this notice are not subject to the clearance procedures of the Office of flanagement and Budget as required by the Paperwork Reduction Act of 1980, PL96-511.
