Report Number: SD 413/81-06 Report Date: June 5, 1981 Facility: Catswha Nuclear Station, Unit 1 Identification of Deficiency: Due to isolated human error, four 2" 3000#
cpecial flow orifice couplings and forty-five 2" X 3/4" special flow orifice reducing couplings were ordered and designated as 6000# fittings. Two of the 2" and 15 of the 2" X 3/4" fittings were assigned to systems which require 6000# fittings on construction drawings. All but four of the assigned fittings were installed.
Initial Report: On May 8, 1981, Mr. J. Bryant of NRC Region II, Atlanta, Ga was notified of this deficiency by Messers W. O. Henry, D. M. Collings and R. L. Williams of Duke Power Co., Charlotte, N.C. 28242. This notification was as a result of Potentially Reportable Item CA-81-11.
Supplier and/or Component: This report covers fittings manufactured to Duke dravings and is not a supplier related problem.
Description of Deficiency: Four 2" special flow orifice couplings and forty-five 2" X 3/4" special flow orifice reducing couplings were ordered on requisitions CN 1206.00-02-0052 and 8337-1206.00-02-0843 as 6000# fittings per drawing CN-1680-5 and CN-1680-6 respectively.
Drawings CN-1680-5 and CN-1680-6 did not specify the pressure class and a later dimensional review revealed the fittings met 3000# pressure rating but not 6000# pressure ratinr;.
Analysis of Scfety !=plications: Had these fittings gone undetected, they would not have led to system failure. Maximum design conditions for systems for which these were ordered are 2500 psig at 650 F.
The fittings as produced are good for design conditions of 2764 psig at 650 F.
End reruit would have been reduced design conservatism.
Corrective Action: A review of all the piping material inventory control system allocations was conducted to assure that no other misapplication of special fittings exists.
One additional deficiency was identified.
Drawing CN-1491-NC022 which requires 6000# fittings specified a fitting per drawing CN-1680-6. This drawing has been revised to show the applicable 6000#
fitting.
In addition the following action was taken to correct the subject deficiency:
- 1) The installed 3000# fittings were cut out, scrapped and replaced with 6000# fittings.

- 2) The remaining stock fittings were remarked as 3000#.
- 3) Drawings CN-1491-N1079, N1084, N1003, N1004, N1081, NC043, NC048, NC053, NC099, NC119, NV076, NV078, NV082, and NV083 were revised to show the applicable 6000# fittings.

To prevent future reoccurrence of a similar deficiency, the piping specification and all series 1680 drawings for special dimensioned fittings will be revised to show the applicable pressure class.
Revision will be completed by August 1, 1981.
