The requirements of Table 3.2.A for main steam' isolation valve high-temperature isolation were not met. Both channals of foolation lo5 c i
were jtmpared and rendered inoperahla. The condition was recognized, and the jumpers removed within 48 minutes of being installed.
Apparent Cause of,0ccurrence Personnel ^ error in interpreting plant procedures concerning emergency use of temporary alterations.
Other Related Events None Co_rrective Action Taken or Planned The jtapers were removed within 48 minutes of being installed.
Investigation la presently underway to determine what personati actions and/or procedural changes are necessary to prevent rec _urrence.
