Appendix A NOTICE OF VIOLATION Louisiana Power and Light Company Docket: 50-382 CPPR-103 As a result of the inspection conducted on July 6-10, 1981, and in accordance with Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violation was identified:
Failure to Follow Procedures Criterion V of Appendix B to 10 CFR Part 50 requires that activities affecting quality shall be prescribed by documented procedures of a type appropriate to the circumstances, and shall be accomplished in accordance with these procedures.
The following two examples of failure to follow procedures were noted:
1.
Section 8.0 to Division 4 of the Cliicago Bridge and Iron (CB&I)
Nuclear Quality Assurance M.inusi for ASME Section III Products, Issue No. 6 requires that d I unused welding electrodes shall be returned to the storage cvens or discarded (destroyed) at the end of each shift.
Contrary to the above:
On July 6,1981, the NRC inspectorsobserved several unused welding electrodes in a bucket and on the floor in the CB&I work area that had not been returned to the storage ovens, destroyed, or removed from the work area and discarded after the end of the work shift.
2.
Section 14.0 to Division 4 of the Chicago Bridge and Iron Nuclear Quality Assurance Manual, Issue No. 6 requires that all noncon-formities awaiting resolution shall be entered on the Nonconformance Control List and repairs shall be controlled aa' documented on a Repair Check List.
Contrary to the above:
On July 7,1981, the NRC inspectors determined that reinstallation of the Containment Equipment Hatch Cover was classified by Chicago Bridge and Iron as a nonconformance/ repair which was not entered on the Nonconformance Control List nor controlled and documented on a Repair Check List.
This is a Severity Level V violation.

Pursuant to the provisions of 10 CFR Part 2.201, Louisiana Power and Light Company is hereby required to submit to this office, within 30 days of the date of this Notice, a w"itten statement or explanation in reply, including:
(1) the corrective stepi which have teen taken and the results achieved; (2) corrective steps which will be taken to avoid further violations; and (3) the date when full compliance will be achieved.
Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or affirmation.
Consideration may be given to extending your response time for good cauce shown.
The response directed by this Notice is not subject to the clearance proce-dures of the Office of Management and Budget as required by the Paperwork Reduction Act of 1980, PL 96-511.
