1.0 PURPOSE The purpose of this procedure is to establish the requirements and methods for controlling activitie's' that temporarily
- alter the design' fu'nc' tion of a syst' m-e or component. The requirements of this procedure apply to all activities.that temporarily alter the design functi.on of any component or system.
Methods used to document temporary alterations constructed under approved test, maintenance or surveillance procedures are contained in section 6.6 of this procedure.

2.0 RESPONSIBILITY 2.1 The Shift Supervisor is responsible for approving all system temporary alteration requests af ter. reviewing-them to determine the actual need, a
the affect on plant operation, and reactor safety requirements in accordance with GGNS Technical Specifications.
2.2 The Control Room Operator is responsible for reviewing all system temporary alteration requests for affect on plant operation and reactor safety requirements in accordance with GGNS Technical Specifications.
2.3 An Independent Verifier shall verify proper placement and removal of all system temporary alterations on safety related sy tems.

,. g 5.0 DEFINITIONS 5.1 System Temporary Alteration 5.1.1 A change that inhibits or alters the intended operation of a plant '
component or system such as electrical jumpers, lif ted wires, piping blocks and bypasses, papered contacts, or temporary setpoints that are intended te be returned to normal or permanently incorporated at some later date and not, otherwise covered by an approved procedure which returns the system back to-normal.
5.1.2 Any system modification designated by the Shift Supervisor as a System Temporary Alteration.
5.2 Electrical Jumper - A wire or cable connected across terminal (s) which is not in accordance with drawing or procedure.
5.3 Lifted Wire - A break in continuity of,a circuit where one connection of a wire previously connected between two terminals is removed.

5.4 Piping Block - A flange, plug, or other restriction installed in a pipe

to prevent or reduce flow.
5.5 Piping Bypass - Pipe or tubing installed that allows flow. to bypass a component or a device.
i 5.6 Papered Contacts - Contacts in relays or knife switches that have been electrically insulated such that if the switch or relay was closed, the contact would not make electrically.
5.7 Temporary Setpoints - The adjustment of a component or circuit to cause a device to actuate at a setpoint other than that listed in approved permanent plant documents, or to prohibit the normal auccaatic control

.5.12 Independent Verifier - An Operator designated by the Shif t Supervisor to verify proper installation or removal of a temporary alteration on safety related systems.
If, in the Shif t Supervisor's judgement, the Operator is not technically qualified to perform the verification, the Shift Supervisor will also designate a person from the Maintenance Section who is technically qualifed and not responsible for installing the alteration, to assist the Operator as the Independent Verifier and both will sign as Independent Verifiers.
During the Pre-operational, Acceptanc.e Teat Phase, the Independent. Verifier may be the Startup Test Supervisor for alterations controlled in accordance with section 6.6.2 of this procedure.
6.0 DETAILS 6.1 General 6.1.1 Requests for a system temporary alteration shall be made to the Shif t Supervisor on the Temporary Alteration Request Form h
(Attachment I).
6.1.2 A Temporary Alteration Tag shall be assigned and normally attached to each temporary alteration requiring a tag.
However, it is permissible to attach a single tag to more than one lifted wire or jumper if the wires or jumpers can be physically bound together and identified by one tag and if the wires are color coded or can otherwise be easily identified.
6.1.3 When necessary, Danger Tags will be placed on affected equipment in accordance with Reference 3.4.
Temporary Alteration Tags are never to be.used in place of Protective Tags.
6.1.4 The Shift Supervisor shall review and approve all temporary alterations.
He shall give proper consideration to the following:

6.2 Guidelines for Consouction and Installation of Temporary Alterations

6. 2'.1 several wire sizes larger or of the same size but with heavier (larger diameter) outer covering than the majority of wire in the location of the temporary jumper.
The jumper will be long enough _
to allow approximately 6 inches of excess wire. The jumper must not be dressed or tied or run in back of any other wire.
The-connections must be secure.
The tag shall be secured in the middle of the jumper.
6.2.1 Lifted leads will be pulled out away from the terminal strip -

the wire, bcck fra the idancif-i-cation-numbera, where-poesiblenth--purple--colored-tape 6.2.3 Where possible, blocks shall be made obvious, such as a blind flange installed in the middle of a piping run shall have a protrusion from the flange bolt circle, (frying pan handle) painted purple, and plugs inside pipe ends shall have handles protruding out of the pipe, preferably secured to prevent the plug SN from being shoved or drawn further into the pipe and lost.
Insulation must not be installed on a flange with a frying pan handle since it would hide the protrusion.
6.2.4 Hoses or temporary piping that may carry borated. water or othe-solutions that may solidify shall be heat traced if the ambient temperature may drop low enough to cause precipitation.
6.2.5 Papering of contacts either in knife switches or relays will be done using a suitable insulating material.
The paper will be installed leaving tails at least 6 inches long hanging from the contact switch blade.
Relays will not be covered with contacts left papered.
6.3 Requesting and Initiating a System Temocrary Alteration 6. 3.1 - A Job Supervisor may-requese a system temporary alteration.by '
obtaining a Temporary Alteration Form and filling out the
. applicable parts as desc'ribed on Attachment II.
6.3.2 The Supervisor shall submit the applicable reference drawings detailing the work to be accomplished, the Procedure, Trouble Ticket or Maintenance Work Order under which the work is to be performed, along with the Temporary Alteration Form to the Shif t Supervisor for approval.

O 6.3.3 The Shift Supervisor shall:

Review the Temporary Alteration. Request for:

(2) Technical Specification Requirements (3) Plant Operational Problems and any special operating conditions which must be established.
b.
Review the Reference Drawing Procedure, Trouble Ticket or Maintenance Sork Order for completeness and the scope of the work to be performed.
c.
App te the request by placing his signature, date and time on the Temporary Alteration Request Form.
d.
Assign an operator and, if necessary, a maintenance representative to provide independent verification if the alteration is safety related.
e.
Forward the Temporary Alteration Request, Reference Drawing Procedure, Trouble Ticket or Maintenance Work Order to the Control Room Operator.
6.3.4 The Control Room Operator shall:
a.
Review the temporary Alteration Request for Technical Specification requirements, plant operational problems, and any special operating conditions which must be established.
b.
Concur with the Temporary Alteration by affixing his signature, date, and time.
c ".
Log the information and appropriate log numbers for each alteration in the Temporary Alteration Log (Attachment IV).
d.
Initiate the appropriate actions to establish any special operating conditions which must be established as a result of the temporary alteration.

On safety related systems, the Independent Verifier verifies that the alteration is properly performed. The person performing the work and the person (s) performing independent verifications, if applicable, shall sign the Temporary Alteration Sheet.
6.4 Continuation of a Temporary Alteration through Shift Change 6. 4'.1 If the Temporary iteration is for a surveillance test or preventive maintenance instruction which extends over the shif t on which the work was authorized to cocmence by the Shif t Supervisor, the work supervisor will:
Notify the Shif t Supervisor of the job status and conduct a a.
turnover on the job location with continuous surveillance, or b.
Restore the Temporary Alteration in accordance with the requirements of this. procedure.
6.5 Restoration of System Temporary Alterations 6.5.1 When restoration is desired, Maintenance shall use the Temporary Alteration Form to restore the system to the design condition; remove and destroy any Temporary Alteration Tags, and sign the Temporary Alteration Form.
For safety related systems, removal of the alteration and restoration of the system shall also be verified by an Independent Verifier designated by the Shift Supervisor, who will sign the Temporary Alteration Form.
The Temporary Alteration Form shall be returned to the Control Room Operator.
r 6.5.2 The Control Room Operator shall review the Temporary Alteration Form for completeness; affix his signature, date, and time; initiate the removal of Protective Tags if placed; clear the alteration from the Tempo.rary Alteration Log, and forward the, s

completed Temporary Alteration Ferm to the 'Shif t 'S'upervisor.
6.5.3 The 'Shif t Supervisor shall review th'e ' Temporary Alteration Form for completeness and affix signature and date. The Temporary Alteration Form shall be placed in the inactive part of the Temporary Alteration Log.

6.6 Controls for Temporary Alterations placed Under an Aoproved Test Procedure, Maintenance or Surveillance Instruction 6.6.1 Alterations that are performed in accordance with approved maintanance or surveillance instructions require no additional documentation provided the following criteria are met:

FILLING OUT A TEMPORARY ALTERATION SHEET Each Temporary Alteration Sheet shall, provide the following information: -
1.
The name of the system or component affected along with the MP&L equipment number.
2.
The name and title of the person requesting the alteration, and the date
~
requested.
2.
The reason for the alteration.
Include a brief description of the work to be performed, and the physical effect the Temporary Alteration will have on the equipment or system (see example under step 9. below).
4.
The date and/or time the condition is expected to be restored.
s 5.
The Work Document Number that the work is being performed under.
(If Required) 6.
A determination as to whether or not the task is safety related and whether independent verification is required.
s 7.
The type of alteration (jumper, lif ted wire, piping block, or bypass, etc.)
that is to be made.
8.
The physical location of where the alteration is to be made, such as panel number, pipe location.
- 9. 'A description of what the alteration consists of, such as terminal connections,
wire numbers, pipe number, the name and number of an instrument with a temporary setpoint adjus tment as well as the value of the temporary setpoint, etc.
EXAMPLE:
Reason for alteration - remove local control of Auxiliary Steam Feedwater Pump N12C001A by local handswitch SN12 HS-n003A and remove the handswitch for maintenance.
