APPENDIX A NOTICE OF VIOLATION Docket No. 50-160 Georgia Institute of Technology Georgia Tech Research Reactor License No. R-97 As a result of the inspection conducted on April 7-9, 1981, 'and in accordance with the Interim Enforcement Policy, 45 FR 66754 (October 7,1980), the following violations were identified.

A.

Technical Specification paragraph 1.27 requires that a surveillance task be accomplished within plus or minus 25*; of the required interval.

Contrary to the above, the Emergency Core Cooling System (ECCS) monthly surveillance frequency was exceeded in that one surveillance scheduled for October 3, 1980 was actually accomplished on October 24, 1980 and one surveillance scheduled for January 9, 1981 was actually accomplished on January 27, 1981.

Technical Specification 6.4.b.(7) requires that written procedures shall be provided and utilized for surveillance and testing requirements. Technical Specification 6.4.a requires that all procedures and major changes thereto shall be reviewed and approved by the Nuclear Safeguards Committee prior to being effective.

Contrary to the above, as of April 9,1981, surveillance procedures for Control Rod Drop Time and Emergency Core Cooling System Flow Rate Testing do not contain sufficient precautions, prerequisites, directions, and acceptance criteria to accomplish the required surveillance test.

Additionally these and other Technical Specification required surveillance procedures do not evidence the required review and approval of the Nuclear Safeguards Committee.

C.

10 CFR 50.54(1-1) requires the licensee to maintain in effect the NRC approved Operator Requclification Program. The licensee's approved program requires each licensed indiviaual to perform at least fU e control mani-t pulations annually. Additionally the performance and c.smpetency of each licensed individual will be annually observed, evaluated, and documented.

Contrary to the above, during the calendar year 1980, two Senior Reactor -

Operators failed to complete at least five control manipulations and the performance and competency of four individuals was not observed, evaluated,.,,

and documented.

Pursuant to the provisions of 10 CFR 2.201, you are hereby required to submit to this office within twenty-five days of the date of this Notice, a written state-ment or explanation in reply, including: (1) admission or denial of the alleged violations; (2) the reasons for the violations if admitted; (3) the corrective steps rhich have been taken and the results achieved; (4) corrective steps which will be taken to avoid further violations; and (5) the date when full compliance will be achieved. Under the authority of Section 182 of the Atomic Energy Act of 1954, as amended, this response shall be submitted under oath or, affirmation.
