in accordance with the requirements of Title 10, Chapter 1, Fede ral Regulations, Part 21, Transamerica Delaval, Inc., hereby notifies the Commission of a potential defect in a component of a DSRV or DSR Standby Diesel Generator. There exists a potential problem with valve springs which could result in engine non-availability.

Transamerica Delaval has supplied the DSR and DSRV engines to the following sites. Also included in the list are spare parts shipments which had the defective springs.

1) Tennessee Valley Authority Hartsville B, Unit 2 S/N 77030 DSRV16 Hartsville B, Unit 2 S/N 77031 DSRV16 Phipps Bend, Unit 1 S/N 77032 DSRV16 Phipps Bend, Unit 1 S/N 77033 DSRV16 Phipps Bend, Unit 2 S/N 77034 DSRV16 Hcrtsville A, Unit 2 S/N 77026 DSRV16 (1) Spare cylincer head, (4 springs), Hartsville warehouse (4) Spare valve springs - Hartsville B, Unit 2 (4) Spare valve springs - Phipps Bend, Unit 1 2)

Gulf States Utilities River Bend, Unit i S/N 74034 DSR8 River Bend, Unit 1 S/N 74035 DSR8 3)

Carolina Power & Light Shearon Harris S/N 74046 DSRV16 Shearon Harris S/N 74047 DSRV16 None of these units have been placed in commercial operation.

These val.c cprings were installed on the engine cylinder head assembly by Transamerica Delaval.

The potential defect is improperly manufactured valve sprin7s.

Some of the springs delivered to Transamerica Delaval by Melrose Spring and Tool Works were not shot peened oroperly. This improper shot-peening could lead to failure of the vr.ve spring.

If the valve spring fails, engine availability could be affected.

A copy of this letter will be sent to each of the cognizant parties as listed in paragraph 2 no later than August 14, 1981. Transamerica Delaval has in stock sufficient valve springs of the proper quality to replace all of the suspect valve springs. The engine instruction manual contains instructions on how to change valse springs and each site has a copy of the instruction manual. Transamerica Delaval will furnish parts and tech-nical services as required on request and in accordance with each individual contract.

Since all of the sites involved are still in the construction phase, we cannot estimate when the valve spring change out will be completed. The change requires removing the cylinder head cover and the ability to bar the engine over. This can only be done after the engine is installed on its foundation and the flywheel has been installed.

This report confirms an initial telephone report to Mr. Bobby Faulkenberry, Region 5, Walnut Creek, California, on July 30, 1981.

Our evaluation of this matter was concluded on July 29, 1981.
