Dear Ms. Vacca:
St. Luke's Hospital requests a five year renewal of NRC License number SNM-1638. This license presently allows the Hospital to implant PU-238 Nuclear-Powered Cordis Omni-Stanicor Model 184A Cardiac Pacemakers.
The physicians of this institution wish to continue to receive, store, implant, and dispose of the nuclear-powered pacemakers. To this end, the hospital requests that:
1.
The present possession limit (20 individual sources not exceeding 225 milligrams PU-238 each) be continued.
2.
The physicians listed in the original license application be authorized to use this special nuclear material for "implan-tation in humans as a component of Cordis Omni-Stanicor Model 184A nuclear-powered cardiac pacemakers for clinical evaluation purposes."
- 3. The list of authorized physician users be amended by dropping Lowell Davis, M.D., and H. D. Friedberg, M.D.
These individuals are no longer practicing at St. Luke's Hospital.
