SUBJECT: NRC INSPECTION REPORT 50-298/94-03 (NOTICE OF VIOLATION)
This refers to the inspection conducted by R. Kopriva of this office on
January 2 through February 12, 1994. The inspection included a review of
activities authorized for your Cooper Nuclear Station facility. At the
conclusion of the inspection, the findings were discussed with you and those
members of your staff identified in the enclosed report.
Areas examined during the inspection are identified in the report. Within
these areas, the inspection consisted of selective examinations of procedures
and representative records, interviews with personnel, and observation of
activities in progress.
The purpose of the inspection was to determine
whether activities authorized by the license were conducted safely and in
accordance with NRC requirements.
Based on the results of this inspection, certain licensed activities appeared
to be in violation of NRC requirements, as specified in the enclosed Notice of
Violation (Notice). The violation is of concern because it showed a weakness
in your control of manual primary containment isolation valves. Although this
weakness had been identified by members of your staff in evaluation of
Licensee Event Report 93-012, your corrective action to develop a systematic

method to ensure that manual primary containment isolation valves were
properly controlled had not been completed.
Inspectors found additional
examples where manual primary containment isolation valves were not controlled
by approved procedures.
In addition, an unresolved item was opened regarding the acceptability of a
single manual valve for containment isolation purposes. Two examples where
the primary containment isolation valve configuration was not in accordance
with the original design were identified.
In another case, original drawings
showed that two valves were required, yet in many instances only one manual
valve has been provided for primary containment integrity.
You are required to respond to this letter and should follow the instructions
2,pecified in the enclosed Notice when preparing your response.
In your
response, you should document the specific actions taken and any additional
actions you plan to prevent recurrence. After reviewing your response to this
Notice, including your proposed corrective actions and the results of future
