The defect involves a component used in the mam rr':ture of the Amorsham Model 920 camera. The component involved

- hat incorporates the male " quick disconnw t" fitting (Aneromm 9
- h Number 90001-4 ). This fitting is for unnecting tM gu.4 ja tuta. and related accessories.

Thow is a chan 101 on the male fitting th:t, engages the beatings on the quick dlaconnect to allow a secu'/e connection. This chan-nel is not properly located on some of the defective parts. Due to this it is possible thrat the guide tube could become discon-nected from the camera while the source is lit the exposed posi-tion. We feel that this 14 unlikely, but is still possible.

This part histei ically ad been manufactured from an item brought in "of f-the-shel " and then modified. Our drawings only included d: stalled specif.W ons for the portions we modified.
The way this.part is manu 44)ared was changed in December 1987. At that (K.
time we arranged for a local machine shop to manufacture the part f ow, raw stock. The ora'ing we sup catiens for the channel location. plied did not include specifi-We have since tightened our controls-in Engineering so that this type of problem is not likely to re-occur.

4 Page 2 of 3 The first shipment of parts manut.:mtured using this method were roccivee. oec.mber 29, 1982. We havo manuf actured sixt.en (1 3 920 cameras since that time. The serial numbers possibly affected are 51 through 66. Thirtoon of these have been obipped to domontic customora, two to Amersham International in the U.K.,
and one remains in stock (Serial / 66). All domestic customorn have been identified.
In addition wo are checking our service records to datormine if any cameras have boon servicod to include replacement of this part since January 1988.
Corrective actions are as follows:
1.
Contact-all customers and distributorn who havo re-coivod che cameras in question. Tolophone contact was initie'4ed by Amorcham on January 16, 1991. One customer was contacted on January 15, 1991 by NRC Region 4.

Thia gauge can be used by Amersham personnel to check cameras in the field or at the service contors.
The cameras can also be returned to the Burlington facility to be inspected.

All cameran that possean the defective component will be nood to be returned to the Durlington facility for replacement of the fitting.
We will koop records of the telephone contacto, units checked, and units requiring replacement of a defectivo component.

Date called We are calling you to inform you of a possible component defect on a small number of the Model 920 exposure devices. Duc to the nature of the problem, we have classified this as a Part 21 defect and the NRC has been informed.
The defect may prevent a secure connection when the guide tube or related accessory is connected to the front of the Model 920. In most cases the connection is secure. In nosu cases the connection is secure. However, it is possible, but ver/ 'anlikely, that the connection may not be properly secured and the guide tube-could-become disconnected from the 920 while the soutte is within the guide tube, only a small' group of 920 cameras may be affected by this. The serial numbers of the devices in your possession that need to be inspected are:

We are also checking our files to see if any cameras you have sent in for repair need to be checked.
As a corrective action we need to arrange for one of the follow-ing:
- We can arrange for the return of the device (a) in your possession that need to be checked to cur Burling-ton Ma. service conter. We will inspect the device for this defect, and if the defect is found, the component will be replaced.

- We can have your regional Sales Representative visit
-your facility and they can.do a quick and simplo check to determine if your unit has the defective conponent.
If the component is found to be defective, it will have to be returned to our Burlington, Ma. service center for repair.
We will make any transport arrangemente for you. Your camera Will be returned promptly. If you have any special requirements we will try-to accommodate them.

This letter is a follow up to,the notification we made by tele-phone on January 16 and 17, 1991 regarding the Part 21 Notifica-tion for the male fitting on the front of a small number the Amersham Model 920 exposure devices. The NRC has been appropri-ately intornud.
The defect may prevent a nacure connection when the guide tube or related accessory is connectea to tne Iront or une Mocal vzu. In most cases the connection is secure, llowever, it is possible, but very unlikely, that the connection may not be properly secured and the guide tube could become disconnected from the 920 while the source is within the guide tube.
Only a small group of 920 cameras may be affected by thin. The serial numbers of the devicea in your possosalon that need to be inspected are:
