Dear Ladies and Gentlemen:
We would like to renew our Special Nuclear Materials License No. SNM-1558.
Enclosed is a check in the amount of $400.00 to cover the renewal fee.
We would like to continue to operate under our current license with the exception of the device and personnel changes noted below.
We will use and possess the licensed material in accordance with statements, representations and procedures contained in our letters dated April 3, 1980, October 11, 1982, December 27, 1984, April 25, 1985 and February 12, 1990, applicable NRC regulations and license conditions.
The devices referenced in application dated January 15, 1986 have been returned to supplier and should no longer be reflected in our license.
We would like to add Messrs. Richard E. Kangas and Kenneth P. Rancourt to Condition 12 of the license.
Formal radiation safety training and experience for these individuals is indicated in duplicate on the enclosed.
We would also like Mr. Richard A. Bauer dropped from Condition 12.
Financial Assurance for Decommissioning (Regulatory Guide 3-66) was reviewed and it was determined that financial assurance was not required.
