a Duke Power Company Catawba Nuclear Station Report No.: SD 414/82-25 Report Date: January 28, 1983 Facility: Catawba Nuclear Station Unit 2 Identification of_ Deficiency: Incomplete penetration has been identified on Class 3 (ASME Section III Subsection ND) circumferential butt welds by welder

  1. 600. These joints were stainless steel welded with the GTAW process, and did not require radiographic examination. Prior visual inspection and NDE of the outside exposed surface indicated these welds were acceptable.

Initial Report: On November 22, 1982, Mr. J. C. Bryant of NRC, Region II, Atlanta, Georgia, was notified of this deficiency by Mr. W. O. Henry, Mr. S. H. Van Malssen, and Mr. R. L. Williams of Duke Power Company, Charlotte, North Carolina 28242. This notification was in response to Nonconforming Item No. 15745.

Descrirition of Deficiency: Upon removal of two (2) temporary caps in the Unit 2

, Refueling Water System (KF), the roots of three (3) Class 3 welds made by welder

  1. 600 became accessible for visual examination. They were discovered to have extensive lack of penetration on the root side (weld prep on the I.D. had never been consumed). Ten (10) other welds made by the same welder were cut and examined. This problem is a result of welder #600 failing to maintain Duke Power Company's desired level of quality.

Analysis of Safety implications: The evaluation of safety implications due to the lack of penetration on these welds is in progress. Note that all Class 1 and 2 welds are examined by radiography; therefore, this condition is not present in Class 1 and 2 systems. A complete evaluation will be forwarded upon completion of the corrective action indicated below.

Corrective Action: All GTAW performance qualifications of welder #600 were removed when this problem was identified. Upon further investigation, welder

  1. 600's employment with Duke Power was terminated.

Additionally, we have issued process control to have the entire population of welder #600's Class 3 open butt welds, that did not receive a visual inspection of the I.D., radiographed and interpreted in accordance with our standard pro-cedures for Class 1 and 2 welds. All rejectable indications will be repaired per ND-4000.

Further corrective actions may be initiated as a result of our continuing investigation and analysis of this problem. A full report will be submitted on this problem on or before October 1,1983.
