APPENDIX A Nuclear Valve Division Borg Warner Corporation Docket No. 99900289/82-03 NOTICE OF NONCONFORMANCE Based on the results of an NRC inspection conducted on November 15-18, 1982, it appears that certain of your activities were not conducted in accordance with NRC requirements as indicated below:
Criterion V of Appendix B to 10 CFR Part 50 states: " Activities affecting quality shall be prescribed by documented instructions, procedures, or drawings, of a type appropriate to the circumstances and shall be accomplished in accordance with these instructions, procedures, or drawings.
Instructions, procedures, or drawings shall include appropriate quantitative or qualitative acceptance criteria for determining that important activities have been satisfactorily accomplished."
Nonconformances with these requirements are as follows:
A.
Paragraph 3.6.1 in Section 3 of the Nuclear Products Quality Assurance Manual states, in part, "All drawings and associated documents... generated... shall be reviewed and signed, to signify approval by:
"the Design Engineering Manager; "a Project Engineer who is not the original designer of the product.
Contrary to the above, source control drawings for six 4-inch gate valves were completed and sent to Procurement and Quality Control without being checked by either the Design Engineering Manager or an engineer who was not the original product designer.
B.
Paragraph 4.3.3 in Section 4 of the Nuclear Products Quality Assurance Manual states, in part, " Purchase Orders for Code and Critical material and items and quality services shall be placed only with vendors who are on the Nuclear Products Approved Vendor List."
Contrary to the above, purchase orders for calibration services were placed with three vendors who were not listed on the Nuclear Products Approved Vendor List.
