This refers to the routine safety inspection conducted by Messrs. P. Swetland,
A. Cerne and J. Wiggins of this office on October 6 - November 26,.1982 of
activities authorized by NRC License No. DPR-36 and to the discussions of our
findings held by Mr. P. Swetland with Mr. E. Wood of your staff at the con-
clusion of the inspection.
Areas examined during this inspection are described in the NRC Region I
\\(
Inspection Report which is enclosed with this letter. Within these areas,
the inspection consisted of selective examinations of procedures and representa-
\\
tive records, interviews with personnel, and observations by the inspector.

Based on the results of this inspection, it appears that certain of your
activities were not conducted in full compliance with NRC requirements, as set
forth in the Notice of Violation, enclosed herewith as Appendix A.
These
violations have been categorized by severity level in accordance with the NRC
Enforcement policy 10 CFR 2, Appendix C) published in the Federal Register
Notice (47FR9987)(datedMarch9,1982.

You are required to respond to this
(
letter and in preparing your response, you should follow the instructions in
Appendix A.
In addition to the need for corrective action regarding these specific vio-
lations, we are concerned about the implementation of your quality assurance
program which permitted them to occur. With respect to violation B set forth
in Appendix A, the failure of a shock suppressor rod-end bearing indicates
inadequate corrective action associated with a previous occurrence. Further,
regarding violations C and D set forth in Appendix A, apparent misapplication
of quality control measures to contracted services resulted in violations
which were based upon a sampling of such activities. Consequently, in your
reply, you should describe in particular, those actions taken or planned to
improve tho offectiveness of your quality assurance program. These should
inclede measures planned or taken to ensure prevention of further snubber
bearing failures and to verify thtt inadequate control of contracted services
is not more widespread.
