FINAL REPORT - DER 82-38 DEFICIENCY EVALUATION 50.55(e) ,

ARIZONA PUBLIC SERVICE CONTANY (APS)

PVNGS UNIT NO. 3 I. Description of Deficiency During Recieiving Inspection, four (4) Containment Purge System Isolation 8" Butterfly Valves, supplied by Henry Pratt, were received at the jobsite with loose Limitorque actuator-to-gear box mounting bolts. These bolts were initially secured by Limitorque and not usually loosened by Pratt during assembly to the valves; however, evidence of marred paint on the bolt heads indicates that the bolts were loosened sometime after release from Limitorque. One (1) bolt from each-of two (2) valve actu-ators were found not installed and lying on the shipping pallet, and there was evidence of damage to the shipping container (s).

Contact with Limitorque disclosed that a specific torque value was not specified for the four (4) hex head bolts securing the unit; however, per Limitorque Telex dated June 3, 1982, the bolts should be tightened to approximately 85 ft. lbs. or as tight as reasonably possible since direct access with a torque wrench is not possible.

II. Analysis of *Saf'ety Implications Had this condition remained uncorrected, the valves would not have satisfied required Seismic I design criteria, and c...d fail to close following a SSE during a power access purge operation.

Given a simultaneous LOCA, an excess release of radioactivity from the Containment could occur.

Additional surveillance and investigation indicate that this is an isolated case of an obvious defect detected and corrected as a normal Receiving Inspection function.

A jobsite Quality Assurance Surveillance of eighteen (18) similarly (elivered valves was conducted for evidence of loose bolts, and none were found. A Surveillance by Bechtel's Supplier Quality organiza-tion at the vendor failed to find any other similar problem. This condition is not a generic Deficiency and is evaluated as Not Report-able.

1. Nonconformance Report No JX-642 will be dispositioned to repair the condition by tightening the bolts to approxi-mately 85 ft. Ibs.
2. Bechtel's Senior Project Supplier Quality Supervisor has issued an Inspection Check Sheet for Limitorque Motor-Operated Valves. This request will intensify the inspection activity at all suppliers providing valves with Limitorque operators.

Dear Mr. Stubblefield:

In response to your letter of July 26, the mounting bolts connecting the Limitorque SMB-00 motor to the HBC operator should be torqued down to 85 foot pounds. This value is based on unlubricated fasteners. There are no special tools required as the fasteners are standard socket head cap screws.

This connection is made at the Limitorque factory, and operators are inspected visually to insure all of the hard-ware is tightened. At Pratt factory, the HBC, with SMB at-tached, is mounted to the valve adaptor plate. The entire assembly is then performance tested to assure proper operation and fastener connections. It is both Pratt and Limitorque experience, that inspection procedures and per-formance testing are effective in preventing equipment from being shipped with loose fasteners.

It is noted on Bechtel nonconformance report JX-642 that equipment arrived at jobsite in damaged crates. Loose hardware identified may be the result of rough handling after shipment from Pratt.

This is an isolated case and is not considered reportable under 10CFR Part 21.
