On August 5, 1982, Mr. T. D. Geib of Public Service Company of Indiana, Inc. (PSI) notified your office of a potentially reportable item as required by 10 CFR 50.55(e).. During a field modification / inspection, Westinghouse discovered six sheared pinion keys in Limitorque Model SBO valve motor operators. The sheared pinion keys could result in the failure of the affected valves to perform their intended safety function. This operator is used on a number of valves in safety-related systems supplied by Westinghouse.

Metallurgical evaluations of the sheared pinion keys have indicated that the keys were made of a resulfurized carbon steel (1100 series) in lieu of the required 1018' carbon steel material. Additionally, Limitorque has conducted i
extensive stall tests of the SB-9-25 motor operator with the 1918 pinion key material and verified the adequacy of this material for its intended use.
j As a result of the Limitorque and Westinghouse evaluations, Westinghouse will supply PSI with a list of affected motor operators and provide pinion i
keys fabricated from the required 1018 carbon steel material for replacement l
of those keys in question.
This letter is intended to fulfill the requirements of a final report as-required by 10 CFR 50.55(e).
